肝腺瘤分型回顾学习附1例病例报告

Retrospective analysis on the type of hepatic adenoma:a case report

目的提高对肝腺瘤(HCA)的认识、诊断率。方法结合1例HCA患者的临床资料分析及文献复习,回顾学习HCA的发病原因、影像学鉴别手段、诊断及治疗方法。结果HCA临床表现、肿瘤指标及影像学表现均不具有典型性,术前确诊难度较大,目前诊断依旧是以病理为金标准,针对术前诊断不能明确者可进行多学科讨论,有助于提高HCA诊断率。结论肝腺瘤四种分型具有不同的特点,术前可以进行多学科讨论(Multidisciplinary Team,MDT),结合肝腺瘤的影像学表现,可为术前的临床诊断及后续治疗手段提供参考。

Objective To improve the understanding and diagnosis rate of hepatic adenoma(HCA).Methods Combined with the clinical data analysis of a case of HCA and literature review,the pathogenesis of HCA,imaging identification methods,diagnosis and treatment methods were reviewed.Results The clinical manifestations,tumor indicators,and imaging manifestations of HCA are not typical,and it is difficult to make preoperative diagnosis.The current diagnosis is still based on pathology which was used as the gold standard.Multidisciplinary discussions can be conducted for those whose preoperative diagnosis is not clear,to improve the diagnosis rate of HCA.Conclusions The four types of liver adenoma have different characteristics.Multidisciplinary team(MDT)could be conducted before operation,combined with the imaging findings of liver adenoma,it could provide references for preoperative clinical diagnosis and subsequent treatment.
