抗病毒药物在治疗慢乙肝相关性肝细胞癌应用与进展

摘要 肝细胞癌是当下威胁人类健康的“头号杀手”,主要分布在发展中国家。它是肝癌主要的组织学类型。乙型肝炎病毒(HBV)感染是诱发肝细胞癌的主要因素。据新的研究数据显示病毒学突破(VBT)是肝硬化患者肝癌的独立危险因素,当下,慢乙肝患者急需高效的抗病毒治疗法,以维持病毒抑制,降低肝硬化患者基因病变的风险。恩替卡韦(ETV)和替诺福韦(TDF)具有的低耐药性也是毋庸置疑的,目前已取代替比夫定、阿德福韦和拉米夫定成为患者的一线口服药物。本综述总结了ETV及TDF在慢乙肝导致的肝细胞癌治疗中的重要性;HBX蛋白对肝细胞癌癌的影响;TGF-β1对慢乙肝导致肝细胞癌的风险性;炎性小体在肝细胞癌进展中的作用;自噬在乙型肝炎病毒导致的相关肿瘤发生着双重作用。本文总结了这两种抗病毒药物在治疗慢乙肝转变为肝细胞癌的风险性对比及对未来抗病毒药物发展进行展望。
