基于医院信息系统的真实世界的注射用血栓通(冻干)临床用药特征分析

目的:了解真实世界中注射用血栓通(冻干)的临床用药特征,为规范其临床应用提供参考。方法:基于医院信息系统,选取医院2015年8月—2016年2月使用注射用血栓通(冻干)治疗的501例住院患者,对他们的一般资料及其诊断、临床用药、合并用药和药物不良反应情况进行统计分析,并利用Apriori算法进行合并用药关联分析。结果:501例患者中有男性289例(57.68%)、女性212例(42.32%),平均年龄为70.82岁,其中≥60岁的患者有398例(79.44%)。对所有患者,注射用血栓通(冻干)均以经静脉滴注方式给药,单次剂量以450 mg为主(359例,71.66%),连续给药时间多为1~14 d(473例,94.41%),平均为8.5 d。最常与注射用血栓通(冻干)合并使用的药品依次为阿司匹林肠溶片、阿托伐他汀钙片和硫酸氢氯吡格雷片。结论:注射用血栓通(冻干)的临床用途和用法用量基本符合其药品说明书要求,但仍存在超说明书用药问题;合并用药情况较为合理,但也存在药物相互作用问题,临床上应予重视。

Objective:To study the characteristics of clinical application of intravenous Xueshuantong(lyophilized)in real-world,and to provide reference for standardizing its clinical medication.Methods:Based on the hospital information system,501 cases of inpatients who received intravenous Xueshuantong(lyophilized)from Aug.2015 to Feb.2016 were selected and their general information,diagnostic information,clinical drug use,drug combination information and adverse drug reaction were analyzed by statistical software and the association analysis of combined medication was performed by Apriori algorithm.Results:Two hundred and eighty-nine cases were male(57.68%),212 cases were female(42.32%),the average age was 70.82 years old and most of patients were over 60 years old(398 cases,79.44%).All patients were administered by intravenous drip with a single dose 450 mg(359 cases,71.66%).Course of treatment was mainly 1-14 days(473 cases,94.41%)with an average of 8.5 days.The drugs most commonly used in combination with intravenous Xueshuantong(lyophilized)were aspirin entericcoated tablets,atorvastatin calcium tablets and clopidogrel bisulphate tablets.Conclusion:The clinical use,usage and dosage of intravenous Xueshuantong(lyophilized)were basically meet the requirements of its drug instructions,but there is still a problem of off-label medication.The combination of drugs is more reasonable,but there are also drug interaction problems,to which attention should be paid in clinic.
