对比分析海鸥形锁定钢板和双重建钢板两种内固定方式治疗创伤性踝关节炎的效果

Comparative Analysis of the Effects of Two Internal Fixation Methods of Seagull-shaped Locking Plate and Double Reconstruction Plate in the Treatment of Traumatic Ankle Arthritis

目的对比分析海鸥形锁定钢板、双重建钢板两种内固定方式对创伤性踝关节炎的治疗效果。方法选择2017年1月-2020年4月期间于该院以内固定术治疗创伤性踝关节炎的89例患者,回顾分析其资料,以患者采取的不同内固定方式为分组依据,将其分为实行双重建钢板治疗的对照组(n=49)、实行海鸥形锁定钢板治疗的研究组(n=40)。组间对比术中出血量、手术时间、愈合时间、并发症发生率、远期疗效。结果研究组的总有效率为75.00%,比对照组的51.02%高,差异有统计学意义(χ^2=5.364,P<0.05);研究组并发症发生率为2.50%,比对照组的20.40%低,差异有统计学意义(χ^2=6.520,P<0.05)。研究组的术中出血量、手术时间、愈合时间分别为(99.86±28.36)mL、(59.41±28.37)min、(109.47±9.36)d,对照组分别为(101.04±24.24)mL、(60.24±23.15)min、(111.19±7.04)d,两组较为相近,差异无统计学意义(t=0.212、0.149、0.989,P=0.833、0.882、0.325)。结论相较于双重建钢板内固定,海鸥形锁定钢板对创伤性踝关节炎有更加显著的治疗效果和安全性。

Objective To compare and analyze the therapeutic effects of two internal fixation methods,seagull-shaped locking plate and double reconstruction plate,on traumatic ankle arthritis.Methods From January 2017 to April 2020,89 patients with traumatic ankle arthritis treated with internal fixation in the hospital were selected,and their data were retrospectively analyzed,and the patients were divided into two groups based on the different internal fixation methods adopted by the patients.They were the control group(n=49)with double reconstruction plate treatment and the study group with seagull-shaped locking plate treatment(n=40).The intraoperative blood loss,operation time,healing time,complication rate,and long-term efficacy were compared between groups.Results The total effective rate of the study group was 75.0%,which was higher than 51.02%of the control group.The difference was statistically significant(χ^2=5.364,P<0.05);the complication rate of the study group was 2.50%,which was higher than that of the control group 20.40%,and the difference was statistically significant(χ^2=6.520,P<0.05).The intraoperative blood loss,operation time,and healing time of the study group were(99.86±28.36)mL,(59.41±28.37)min,(109.47±9.36)d,and the control group were(101.04±24.24)mL and(60.24±23.15)min and(111.19±7.04)d,the two groups were relatively similar,and the difference was not statistically significant(t=0.212,0.149,0.989,P=0.833,0.882,0.325).Conclusion Compared with double reconstruction plate internal fixation,seagull-shaped locking plate has a more significant therapeutic effect and safety for traumatic ankle arthritis.
