综合护理对预防下肢骨折术后并发深静脉血栓形成的效果研究

Study on the Effect of Comprehensive Nursing on Preventing Deep Vein Thrombosis after Lower Extremity Fracture Operation

目的观察下肢骨折术后患者应用综合护理对预防患者并发深静脉血栓形成的临床应用效果。方法2018年1月-2020年1月间从该院接受的下肢骨折手术患者中抽选出82例作为研究对象,按随机数表法分为两组,一组接受常规护理(对照组,41例),另一组接受综合护理(观察组,41例)。对比两组凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、血小板计数等凝血功能指标变化、深静脉血栓及下肢肿麻发生情况。结果干预后,观察组PT(16.48±2.59)s、APTT(34.63±3.35)s、PLT(156.48±38.76)×109/L,相比于对照组差异有统计学意义(t=5.670、6.477、5.934,P<0.05)。观察组发生深静脉血栓1例(2.44%)、下肢肿麻2例(4.88%),均少于对照组,差异有统计学意义(χ^2=4.493、6.248,P<0.05)。结论综合护理应用于下肢骨折手术患者术后并发深静脉血栓形成的预防工作,有助于优化患者凝血功能,降低患者深静脉血栓形成风险。

Objective To observe the clinical application effect of comprehensive nursing for patients with lower extremity fractures in the prevention of deep vein thrombosis.Methods From January 2018 to January 2020,82 cases of patients undergoing lower limb fracture surgery in the hospital were selected as the research objects,and were divided into two groups according to the random number table method.One group received routine care(control group,41 cases),the other group received comprehensive care(observation group,41 cases).The changes in coagulation function indexes such as prothrombin time(PT),activated partial thromboplastin time(APTT),platelet count,and the occurrence of deep vein thrombosis and lower extremity numbness were compared between the two groups.Results After intervention,the observation group had PT(16.48±2.59)s,APTT(34.63±3.35)s,and PLT(156.48±38.76)×109/L.Compared with the control group,the difference was statistically significant(t=5.670,6.477,5.934,P<0.05).Deep vein thrombosis 1 cases(2.44%)and lower limb numbness 2 cases(4.88%)in the observation group were less than those in the control group,and the difference was statistically significant(χ^2=4.493,6.248,P<0.05).Conclusion The application of comprehensive nursing care in the prevention of postoperative deep vein thrombosis in patients with lower limb fracture surgery can help optimize the patient's blood coagulation function and reduce the risk of patients with deep vein thrombosis.
