复方黄柏液对肛周脓肿合并糖尿病患者Ⅰ期根治手术后创面愈合的影响

Effect of Compound Huangbai Liquid on Wound Healing of Patients with Perianal Abscess and Diabetes after Stage Ⅰ Radical Surgery

目的探讨复方黄柏液对肛周脓肿合并糖尿病患者Ⅰ期根治手术后创面愈合的影响。方法研究对象为2017年3月-2019年3月泉州市中医院肛肠科收治的116例行Ⅰ期根治术后的肛周脓肿合并糖尿病患者。采用随机数字表法将入组对象分为观察组(n=58)和对照组(n=58)。两组均给予血糖控制治疗,观察组采用复方黄柏液纱布换药,对照组采用凡士林油纱换药。比较两组患者术后7 d和术后14 d创面疼痛程度、渗液情况、创面愈合率及空腹血糖值。结果术后7 d,观察组患者的创面分泌物评分和创面VAS疼痛评分均低于对照组,差异有统计学意义(P<0.05)。术后14 d,观察组患者的创面愈合率高于对照组,差异有统计学意义(P<0.05)。两组患者术后7 d和术后14 d的空腹血糖值对比,差异无统计学意义(P>0.05)。结论复方黄柏液对于肛周脓肿合并糖尿病患者I期根治手术后创面有较好的促愈作用,复方黄柏液纱布换药可以减少渗出物、改善疼痛症状,值得临床推广。

Objective To investigate the effect of compound Huangbaiye on wound healing in patients with perianal abscess and diabetes mellitus after radical resection. Methods The subjects of the study were 116 patients with perianal abscess and diabetes who underwent stage Ⅰ radical resection, admitted to the Department of Anorectal, Quanzhou Hospital of Traditional Chinese Medicine from March 2017 to March 2019. The random number table method was used to divide the subjects into observation group(n=58) and control group(n=58). Both groups were given blood glucose control treatment. The observation group was changed with compound cork liquid gauze, and the control group was changed with petroleum jelly gauze. The wound pain degree, fluid leakage, wound healing rate and fasting blood glucose were compared between the two groups of patients on the 7 th and 14 th postoperative day. Results At 7 d after operation, the wound secretion score and wound VAS pain score of the observation group were lower than those of the control group, and the difference was statistically significant(P<0.05). On the 14 th day after operation, the wound healing rate of the observation group was higher than that of the control group, the difference was statistically significant(P<0.05). There was no significant difference in the fasting blood glucose values between the two groups of patients on the 7 th day and the 14 th day after the operation(P>0.05). Conclusion The compound yellow cypress liquid has a better healing effect on the wounds of patients with perianal abscess and diabetes mellitus after stage Ⅰ radical operation. The compound yellow cypress liquid gauze dressing can reduce the exudate and improve the pain symptoms, which is worthy of clinical promotion.
