DR患者黄斑水肿与抗VEGF治疗的相关研究

Correlative Study of Macular Edema in Patients with DR and Anti-VEGF Therapy

目的研究并评价糖尿病视网膜病变(DR)临床应用抗血管内皮生长因子(VEGF)药物治疗的疗效,以及DR患者黄斑水肿与抗VEGF治疗的相关性。方法对该院2019年8月—2020年8月内收治的35例DR患者予以选取,根据随机分配法进行组别划分。对采用抗VEGF治疗的实验组(18例)与采用常规治疗的对照组(17例)的临床效果进行对比。并对两组治疗前后所予测定的最佳矫正视力(BCVA)以及黄斑中心凹厚度(CMT)水平进行对比。结果实验组治疗总有效率明显高于对照组,差异有统计学意义(P<0.05);治疗前,两组BCVA、CMT的测定数值比较,差异无统计学意义(P>0.05);实验组治疗1、3、5个月后的BCVA、CMT改善幅度明显优于对照组,差异有统计学意义(P<0.05);治疗前,两组黄斑水肿程度测定数值比较,差异无统计学意义(P>0.05);实验组治疗1、3、5个月后的黄斑水肿减轻程度明显优于对照组,差异有统计学意义(P<0.05)。结论采用抗VEGF治疗DR患者黄斑水肿的临床疗效更加突出,同时在BCVA、CMT上有明显的改善作用,可有效减轻其黄斑水肿程度,有利于保护视网膜。

Objective To study and evaluate the clinical application of anti-vascular endothelial growth factor(VEGF)drug therapy in diabetic retinopathy(DR),and the correlation between macular edema and anti-VEGF therapy in patients with DR.Methods 35 patients with DR admitted to the hospital from August 2019 to August 2020 were selected and divided into two groups according to the random allocation method.The clinical effects of the experimental group(18 cases)with anti-VEGF treatment and the control group(17 cases)with conventional treatment were compared.The best corrected visual acuity(BCVA)and central foveal thickness(CMT)levels of the two groups were compared before and after treatment.Results The total effective rate of treatment in the experimental group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was not significant difference in the measured values of BCVA and CMT between the two groups(P>0.05);the improvement of BCVA and CMT in the experimental group after 1,3,and 5 months of treatment was significantly better than that of the control group,and the difference was statistically significant significance(P<0.05).Before treatment,the two groups had no statistically significant difference in the degree of macular edema(P>0.05);the experimental group had significantly better reduction of macular edema after 1,3,and 5 months of treatment than the control group,and the difference was statistically significant(P<0.05).Conclusion The anti-VEGF treatment of macular edema in patients with DR has a more prominent clinical effect.At the same time,it can significantly improve BCVA and CMT,which can effectively reduce the degree of macular edema and help protect the retina.
