分析连续性血液净化治疗糖尿病肾病相关重症并发症患者的价值

Analyze the Value of Continuous Blood Purification in the Treatment of Patients with Severe Complications Related to Diabetic Nephropathy

目的分析连续性血液净化治疗糖尿病肾病相关重症并发症患者中的临床价值。方法选取2018年2月—2020年2月收治的72例糖尿病肾病患者,所有患者均存在不同的重症并发症,经保守治疗效果不佳。随机被等分为观察组(接受连续性血液净化)与对照组(接受普通血液净化治疗),每组36例。将两组治疗后的效果、血糖水平、血流动力学水平、肾功能指标进行对比。结果治疗后,相比于对照组(72.22%),观察组的治疗总有效率(91.67%)更高,差异有统计学意义(P<0.05);治疗后,较之对照组,观察组的空腹血糖、餐后2 h血糖、糖化血红蛋白明显低,差异有统计学意义(P<0.05);两组治疗后的平均动脉压相比差异无统计学意义(P>0.05);对照组与观察组治疗后的中心静脉压、心脏指数均降低,其中观察组降低明显,差异有统计学意义(P<0.05);相比于对照组,观察组的尿素氮、肌酐水平明显低,差异有统计学意义(P<0.05)。结论连续性血液净化在治疗糖尿病肾病相关重症并发症患者中具有较好的效果,患者的肾功能得以明显改善,且血流动力学水平稳定,对患者的损伤小,利于治疗后身体的恢复。

Objective To analyze the clinical value of continuous blood purification in the treatment of severe complications related to diabetic nephropathy.Methods Selected 72 patients with diabetic nephropathy admitted from February 2018 to February 2020,all patients have different severe complications,and the effect of conservative treatment was not good.They were randomly divided into observation group(receiving continuous blood purification)and control group(receiving ordinary blood purification treatment),with 36 cases in each group.The effects,blood glucose levels,hemodynamic levels,and renal function indexes of the two groups after treatment were compared.Results After treatment,compared with the control group(72.22%),the total effective rate(91.67%)of the observation group was higher,the difference was statistically significant(P<0.05);after treatment,compared with the control group,the observation group’s fasting blood glucose and 2 h postprandial Blood glucose and glycosylated hemoglobin were significantly lower,the difference was statistically significant(P<0.05);there was not significant difference in the mean arterial pressure between the two groups after treatment(P>0.05);the central venous pressure and cardiac index of the control group and the observation group were reduced after treatment,and the observation group decreased significantly,the difference was statistically significant(P<0.05);compared with the control group,the levels of urea nitrogen and creatinine in the observation group were significantly lower,the difference was statistically significant(P<0.05).Conclusion Continuous blood purification has a good effect in the treatment of patients with severe complications related to diabetic nephropathy.The renal function of the patient is significantly improved,and the hemodynamic level is stable,which causes little damage to the patient and is beneficial to the recovery of the body after treatment.
