彩色多普勒超声用于下肢深静脉血栓血管内血流异常的诊断价值

目的探讨彩色多普勒超声诊断下肢深静脉血栓对准确显示血管内血流异常的临床价值。方法选取医院2017年3月12日至2019年6月20日收治的疑似下肢深静脉血栓患者94例,患者均进行彩色多普勒超声诊断,计算超声诊断正确率。结果彩色多普勒超声对下腔静脉诊断正确率为95.00%,对腘静脉诊断正确率为93.75%,对髂外静脉诊断正确率为93.75%,对股静脉诊断正确率为94.12%,且敏感度为94.52%,特异度为95.24%,漏诊率为5.48%,误诊率为4.76%。同时,患肢血流速度[(7.16±1.38)L/min]低于健肢;血管内径[(11.35±1.28)mm]长于健肢,血管内膜异常(65.96%)及管内回声异常率(67.02%)高于健肢(P<0.05)。结论采用彩色多普勒超声诊断下肢深静脉血栓,可准确显示血管内异常血流、管腔阻塞程度,确定病变范围。

Objective To investigate the clinical value of color Doppler ultrasound in the diagnosis of deep venous thrombosis of lower extremity.Methods From March 12,2017 to June 20,2019,94 patients with suspected deep venous thrombosis of lower extremity were selected.All patients were diagnosed by color Doppler ultrasound,and the correct rate of ultrasound diagnosis was calculated.Results The diagnostic accuracy of color Doppler ultrasound in inferior vena cava,popliteal vein,external iliac vein and femoral vein were 95.00%,93.75%,93.75%,94.12%,respectively.The sensitivity,specificity,missed diagnosis rate and misdiagnosis rate were 94.52%,95.24%,5.48%and 4.76%,respectively.At the same time,the blood flow velocity of the affected limb was(7.16±1.38)L/min,which was lower than that of the healthy limb,the diameter of the blood vessel was(11.35±1.28)mm,which was longer than that of the healthy limb,the abnormal rates of intima(65.96%)and abnormal echo in the tube(67.02%)were higher than those in the healthy limb(P<0.05).Conclusion Color Doppler ultrasound is effective in the diagnosis of lower extremity deep venous thrombosis,which can accurately show the abnormal blood flow in the blood vessels,the degree of obstruction in the lumen,and determine the range of lesions.
