单纯性肥胖症患者外周血Obestatin、Ghrelin与胰岛素抵抗的关系

Relative study of serum obestatin,ghrelin and insulin resistance in human smiple obesity

目的:探讨单纯性肥胖症患者外周血肥胖抑制素Obestatin、Ghrelin的变化以及与胰岛素抵抗(Insulin resistance,IR)的关系。方法:选取我校2017、2018级学生为研究对象,分为体质指数(Body mass index,BMI)≥28㎏·m^-2的单纯性肥胖症组(n=30)和正常对照组(n=20)。分别采用放免法、酶联免疫吸附测定法、血清生化法检测血清Obestatin和Ghrelin、空腹血清胰岛素(Fasting serum insulin,FINS)、空腹血糖。计算胰岛素抵抗指数(Homeostasis model assessment for insulin resistance,HOMA-IR)。结果:与正常对照组相比,单纯性肥胖症患者血清Obestatin、Ghrelin明显降低(P<0.05),HOMA-IR、Ghrelin/Obestatin(G/O)比值明显升高(P<0.05)。相关性分析显示,血清G/O比值与BMI、腰臀比(Waist to hip ratio,WHR)、FINS和HOMA-IR均呈正相关(r=0.556,0.499,0.475,0.491,P<0.05);与高密度脂蛋白胆固醇(High density lipoprotein cholesterol,HDLC)呈负相关(r=-0.501,P<0.01)。结论:血清Obestatin、Ghrelin水平改变以及G/O比值失衡可能参与肥胖和胰岛素抵抗的发生。

Objective:To investigate the changes of obestatin and ghrelin in peripheral blood of patients with simple obesity and their relationship with insulin resistance(IR).Method:The students of grade 2017 and 2018 in our university were selected as the research objects and divided into simple obesity group(n=30)with body mass index(BMI)≧28 kg·m^-2 and normal control group(n=20).Radioimmunoassay,enzyme-linked immunosorbent assay and serum biochemical method were used to detect serum obestatin and ghrelin,fasting serum insulin(FINS)and fasting blood glucose.The homeostasis model assessment for insulin resistance(HOMA-IR)was calculated.Result:Compared with the normal control group,the serum obestatin and ghrelin were significantly decreased(P<0.05),HOMA-IR and ghrelin/obestatin(G/O)ratio were significantly increased(P<0.05).Correlation analysis showed that serum G/O ratio was positively correlated with BMI,WHR,fins and HOMA-IR(r=0.556,0.499,0.475,0.491,P<0.05),and negatively correlated with high density lipoprotein cholesterol(HDLC)(r=-0.501,P<0.01).Conclusion:The changes of serum obestatin and ghrelin and the imbalance of Ghrelin/Obestatin(G/O)may be involved in obesity and insulin resistance.
