二甲双胍对妊娠期糖尿病患者妊娠结局的影响分析

Analysis of the Influence of Metformin on the Pregnancy Outcome of Patients with Gestational Diabetes

目的妊娠期糖尿病若未给予有效治疗干预,可严重影响妊娠结局,对二甲双胍的治疗效果进行探讨。方法该次调查研究控制时间段为2018年1月—2019年12月,方便选取该次实验研究的均为妊娠期糖尿病患者,共60例,根据患者入院时间的奇偶性完成两组的分组工作,分别给予对照组和实验组应用胰岛素和胰岛素联合二甲双胍控制血糖,对比治疗效果。结果治疗前两组血糖变化情况对比差异无统计学意义(P>0.05);治疗后,实验组空腹血糖(5.02±0.08)mmol/L、餐后2 h血糖(6.61±0.08)mmol/L、糖化血红蛋白(6.25±0.39)%优于对照组的(5.25±0.12)mmol/L、(7.34±0.12)mmol/L、(7.06±0.63)%,差异有统计学意义(t=8.735、27.724、5.988,P<0.05)。实验组和对照组产妇妊娠结局比较(16.67%vs 23.33%),差异无统计学意义(χ^2=1.920,P>0.05);实验组和对照组胎儿分娩结局比较(6.67%vs 13.33%),差异无统计学意义(χ^2=0.741,P>0.05)。结论使用二甲双胍治疗妊娠期糖尿病,可获得较为显著的治疗效果,利于血糖改善,可保证分娩安全性。

Objective If effective therapeutic intervention is not given to gestational diabetes,the outcome of pregnancy can be seriously affected.This article mainly discusses the therapeutic effect of metformin.Methods The control period of the investigation and study was from January 2018 to December 2019.The patients who joined the experimental study were 60 cases of gestational diabetes.The grouping work of the two groups was completed according to the parity of the patients'admission time,and the control group and the experimental group were given insulin and insulin combined with metformin to control blood glucose,compared the therapeutic effects.Results There was not statistically significant difference in blood glucose changes between the two groups before treatment(P>0.05);after treatment,fasting blood glucose(5.02±0.08)mmol/L,2 h postprandial blood glucose(6.61±0.08)mmol/L,and glycosylation in the experimental group hemoglobin(6.25±0.39)%were better than the control group(5.25±0.12)mmol/L,(7.34±0.12)mmol/L,(7.06±0.63)%,the difference was statistically significant(t=8.735,27.724,5.988,P<0.05).Compared of maternal pregnancy outcomes between the experimental group and control group(16.67%vs 23.33%),the difference was not statistically significant(χ2=1.920,P>0.05);compared of fetal delivery outcomes between the experimental group and control group(6.67%vs 13.33%),the difference was not statistically significant(χ2=0.741,P>0.05).Conclusion The use of metformin in the treatment of gestational diabetes can achieve a more significant therapeutic effect,which is beneficial to the improvement of blood glucose and can ensure the safety of delivery.
