侧卧位通气与机械振动排痰对ARDS患者呼吸指标、炎症指标的影响

Effects of Lateral Position Ventilation and Mechanical Vibration Sputum Excretion on Respiratory Index and Inflammation Index in Patients with ARDS

目的:探讨侧卧位通气联合机械振动排痰对急性呼吸窘迫综合征(ARDS)患者呼吸指标、炎症指标的影响。方法:将2017年7月-2019年2月笔者所在医院收治的132例急性呼吸窘迫综合征患者作为研究对象,将其随机分为对照组(66例)和观察组(66例)。对照组采用侧卧位通气治疗,观察组采用侧卧位通气联合机械振动排痰治疗。比较两组呼吸指标及炎症指标。结果:对照组C反应蛋白、白细胞计数、降钙素原水平均明显高于观察组,差异有统计学意义(P<0.05)。对照组氧合指数明显低于观察组,且呼气末正压、吸入氧浓度、气道阻力均明显高于观察组,差异有统计学意义(P<0.05)。结论:采用侧卧位通气联合机械振动排痰治疗急性呼吸窘迫综合征,能够有效改善患者呼吸指标、炎症指标,临床应用及推广价值较高。

Objective:To investigate the effect of lateral position ventilation combined with mechanical vibration sputum excretion on respiratory index and inflammation index in patients with acute respiratory distress syndrome (ARDS).Method:From July 2017 to February 2019,132 patients with acute respiratory distress syndrome admitted in our hospital were selected as the research objects.They were randomly divided into the control group (66 cases) and the observation group (66 cases).The control group was treated with lateral position ventilation,while the observation group was treated with lateral position ventilation combined with mechanical vibration sputum excretion.The respiratory index and inflammation index were compared between the two groups.Result:The C-reactive protein,white blood cell count,and procalcitonin level of the control group were significantly higher than those of the observation group,the differences were statistically significant (P<0.05).The oxygenation index of the control group was significantly lower than that of the observation group,and the positive end-expiratory pressure,inhaled oxygen concentration,and airway resistance were significantly higher than those of the observation group,the differences were statistically significant (P<0.05).Conclusion:The lateral position ventilation combined with mechanical vibration sputum excretion in patients with acute respiratory distress syndrome can effectively improve the respiratory index and inflammation index,and the clinical application value and clinical promotion are high.
