摘要
目的:探讨超声检查子宫内膜厚度(endometrial thickness,ET)与卵巢血流对多囊卵巢综合征(polycystic ovarian syndrome,PCOS)诊断与病情监测价值。方法:将2016年2月-2019年2月笔者所在医院收治的75例PCOS患者纳入观察组,选取同期在院体检的50例健康者作为对照组,两组均使用超声检查ET、卵巢血流[卵巢内收缩期峰值血流速度(peak systolic flow velocity,PSV)、血流指数(flow index,FI)、卵巢动脉阻力指数(resistance index,RI)、卵巢动脉搏动指数(pulsatility index,PI)]情况,以及不同停经时间PCOS患者ET、PSV、FI、RI、PI变化情况。结果:观察组ET、PSV均高于对照组,FI、RI、PI均低于对照组,差异均有统计学意义(P<0.05);停经>90 d患者ET、PSV均高于其他停经时间患者,RI均低于其他停经时间患者,差异均有统计学意义(P<0.05),其他停经时间患者ET、PSV、RI水平比较差异均无统计学意义(P>0.05),不同停经时间患者FI、PI比较差异均无统计学意义(P>0.05)。结论:使用超声检查可详细了解到PCOS子宫内膜厚度增加时,其卵巢动脉血液会越丰富,血流速度越快,而动脉阻力会因此减小,动脉搏动减弱,能够为PCOS的诊断与病情检测提供有力的参考依据。
Objective:To explore the value of ultrasound examination of endometrial thickness (ET) and ovarian blood flow in the diagnosis and condition monitoring of polycystic ovarian syndrome (PCOS).Method:A total of 75 PCOS patients admitted to our hospital from February 2016 to February 2019 were included in the observation group,and 50 healthy groups who underwent physical examination in our hospital during the same period were selected as the control group.Both groups used ultrasound to examine endometrial changes,ovarian blood flow[peak systolic flow velocity (PSV),flow index (FI),ovarian artery resistance index (RI),ovarian artery pulsatility index (PI)]situation,and changes in ET,PSV,FI,RI,PI of PCOS at different menopause times.Result:The ET and PSV in the observation group were all higher than those in the control group,while FI,RI and PI were all lower than those in the control group,the differences were statistically significant (P<0.05).The ET and PSV in patients with menopause for more than 90 days were higher than those in patients with menopause of other time,RI was lower than that in patients with menopause of other time,the differences were statistically significant (P<0.05),there were no statistically significant differences in ET,PSV and RI levels in patients with menopause of other time (P>0.05),there were no statistically significant differences in FI and PI between patients with different periods of menopause (P>0.05).Conclusion:Ultrasound examination can be used to understand in detail that when the endometrial thickness of PCOS increases,the ovarian arterial blood will be richer and the blood flow rate will be faster,and the arterial resistance will be reduced accordingly,and the arterial pulse will be weakened,which can be used to diagnose PCOS and provide a strong reference basis for disease detection.
