经阴道超声检测滋养细胞血流信号诊断剖宫产后瘢痕早期妊娠的价值分析

The Value of Transvaginal Ultrasonography in the Diagnosis of Early Pregnancy with Scar after Cesarean Section

目的:探讨经阴道超声检测滋养细胞血流信号诊断剖宫产后瘢痕早期妊娠的价值。方法:抽取2017年7月-2019年3月收治的109例疑似剖宫产后瘢痕早期妊娠患者进行研究,所有患者均行经阴道超声检测,将术后病理结果作为金标准,计算经阴道超声诊断准确率、敏感度、特异度,并分析超声声像图特征。结果:本组109例疑似CSP患者经术后病理结果证实阳性102例,阴性7例,经阴道超声诊断阳性100例,阴性9例,经阴道超声诊断准确率为94.50%(103/109),敏感度为96.08%(98/102),特异度为71.43%(5/7);经阴道超声对团块型、妊娠囊完全在宫腔内或中下段、妊娠囊嵌于子宫瘢痕处诊断准确率分别为100%(26/26)、73.33%(11/15)、100%(61/61)。结论:经阴道超声对滋养细胞血流信号进行检测用于剖宫产后瘢痕早期妊娠中诊断准确率、敏感度、特异度较高,可为临床诊断治疗提供可靠依据,值得应用。

Objective:To explore the value of transvaginal ultrasound in the diagnosis of early pregnancy with scar after cesarean section.Method:A total of 109 patients with suspected early pregnancy with scar after cesarean section admitted from July 2017 to March 2019 were selected for study.All patients underwent transvaginal ultrasound detection.The postoperative pathological results were taken as the gold standard,and the diagnostic accuracy,sensitivity and specificity of transvaginal ultrasound were calculated,and the ultrasonic image characteristics were analyzed.Result:Among 109 suspected CSP patients,102 cases were positive,7 cases were negative,100 cases were positive by transvaginal ultrasound,9 cases were negative.The diagnostic accuracy of transvaginal ultrasound was 94.50% (103/109),the sensitivity was 96.08% (98/102),and the specificity was 71.43% (5/7).The diagnostic accuracy of transvaginal ultrasonography for the mass type,the pregnancy sac completely in the uterine cavity or the middle and lower segment,and the pregnancy sac embedded in the uterine scar was 100% (26/26),73.33% (11/15) and 100% (61/61),respectively.Conclusion:The detection of trophoblast blood flow signal by transvaginal ultrasound in the diagnosis of early pregnancy with scar after cesarean section has high accuracy,sensitivity and specificity,which can provide reliable basis for clinical diagnosis and treatment,and is worthy of application.
