输尿管软镜钬激光碎石术治疗上尿路结石的临床效果

Clinical Effect of Flexible Ureteroscope Holmium Laser Lithotripsy for Upper Urinary Calculi

目的:分析输尿管软镜钬激光碎石术治疗上尿路结石的临床疗效。方法:选取2019年1月-2020年2月北京丰台医院收治的上尿路结石患者96例,按照随机数字表法分为观察组和对照组,各48例。对照组采用经皮肾镜碎石术治疗,观察组采用输尿管软镜钬激光碎石术治疗,两组术后均随访2个月。比较两组术中出血量、手术时间、预后时间、术后30 d结石清除率、术后并发症发生情况及治疗总有效率。结果:观察组手术时间、预后时间短于对照组(P<0.05);观察组术中出血量少于对照组(P<0.05);术后30 d,观察组结石清除率高于对照组(P<0.05);观察组术后并发症发生率低于对照组(P<0.05);观察组治疗总有效率高于对照组(P<0.05)。结论:输尿管软镜钬激光碎石术作为微创手术,治疗上尿路结石可以明显减少手术时间,患者术中出血量少、创伤小、痛苦小、预后快,且结石清除率高,术后并发症少,安全性高,临床效果好,值得推广使用。

Objective:To analyze the clinical effect of flexible ureteroscope holmium laser lithotripsy in the treatment of upper urinary calculi.Method:From January 2019 to February 2020,96 patients with upper urinary calculi admitted to Beijing Fengtai Hospital were selected.According to the random number table method,they were divided into the observation group and the control group 48 cases in each group.The control group was treated with percutaneous nephroscope lithotripsy,the observation group was treated with flexible ureteroscope holmium laser lithotripsy,they were followed up for 2 months after surgery.Compared the two groups with intraoperative blood loss,operation time,prognosis time,calculi clearance rate 30 d after surgery,postoperative complications and the total effective rate.Result:The operation time and prognosis time of the observation group were shorter than those of the control group (P<0.05).The intraoperative blood loss in the observation group was less than that in the control group (P<0.05).At 30 d after operation,the calculi clearance rate of the observation group was higher than that of the control group (P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group (P<0.05).The total effective rate of the observation group was higher than that of the control group (P<0.05).Conclusion:Flexible ureteroscope holmium laser lithotripsy is a minimally invasive surgery for the treatment of upper urinary calculi,which can significantly reduce the operation time,and reduce intraoperative blood loss and trauma,reduce pain,and achieve a quick prognosis,has high stone clearance rate,fewer postoperative complications,high safety,good clinical effect,and is worth promoting.
