B超引导下脊神经根脉冲射频在带状疱疹后遗神经痛中的应用

Application of B-ultrasound Guided Spinal Nerve Root Pulse Radiofrequency in Postherpetic Neuralgia

目的:探讨带状疱疹后遗神经痛患者应用B超引导下脊神经根脉冲射频的治疗效果。方法:选择笔者所在医院2016年12月-2019年10月收治的带状疱疹后遗神经痛患者200例,根据盲抽法将其分为两组,各100例。对照组实施常规药物治疗,在此基础上,观察组加用B超引导下脊神经根脉冲射频,比较两组不同时间段VAS评分、炎症因子水平[白细胞介素(IL)-1β、瘤坏死因子(TNF)-α]及不良反应。结果:治疗结束时、治疗后4、8周及6个月,两组VAS评分均低于治疗前,且观察组VAS评分均低于对照组,差异有统计学意义(P<0.05);治疗后4周,两组IL-1β、TNF-α水平均低于治疗前,且观察组指标均低于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率比较差异无统计学意义(χ^2=1.607,P=0.205)。结论:带状疱疹后遗神经痛患者应用B超引导下脊神经根脉冲射频的治疗效果较好,可显著降低疼痛,缓解炎症状态,改善预后,且安全性较高。

Objective:To investigate the effect of B-ultrasound guided spinal nerve root pulse radiofrequency in patients with postherpetic neuralgia.Method:A total of 200 patients with postherpetic neuralgia admitted to the author’s hospital from December 2016 to October 2019 were selected,and were divided into two groups according to blind extraction,100 cases each,the control group was treated with conventional drugs,on this basis,the observation group was added with B-ultrasound guided spinal nerve root pulse radiofrequency,the VAS scores,inflammatory factor levels (IL-1,TNF-α) at different time periods and adverse reactions were compared between the two groups.Result:At the end of treatment,4,8 weeks and 6 months after treatment,the VAS scores in both groups were lower than those before treatment,and VAS scores in the observation group were lower than those in the control group,the differences were statistically significant (P<0.05).Four weeks after treatment,the levels of IL-1 and TNF-α in both groups were lower than those before treatment,and the indexes in the observation group were lower than those in the control group,the differences were statistically significant (P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups (χ^2=1.607,P=0.205).Conclusion:The patients with postherpetic neuralgia treated with B-ultrasound guided spinal nerve root pulse radiofrequency is effective,which can significantly reduce pain,relieve inflammation,improve prognosis and safety.
