Changes and Significance of D-D,Hcy and hs-CRP Levels in Patients with Acute Cerebral Infarction

摘要 目的:研究急性脑梗死患者D-二聚体(D-D)、同型半胱氨酸(Hcy)、超敏C-反应蛋白(hs-CRP)水平的变化与意义。方法:选取2018年2月-2020年7月笔者所在医院收治的200例急性脑梗死患者作为观察组,另选取同期于本院进行体检的无相关脑血管疾病健康志愿者100例作为对照组。比较两组血清D-D、Hcy、hs-CRP水平;比较观察组中不同梗死灶直径和不同程度神经缺损脑梗死患者血清D-D、Hcy、hs-CRP水平,并采用Spearman相关性分析法分析D-D、Hcy、hs-CRP水平与梗死灶面积、神经缺损程度的相关性。结果:观察组血清D-D、Hcy、hs-CRP水平均高于对照组,差异均有统计学意义(P<0.05);大梗死组血清D-D、Hcy、hs-CRP水平均高于中梗死组及小梗死组,中梗死组D-D、Hcy、hs-CRP水平均高于小梗死组,差异均有统计学意义(P<0.05);重度组血清D-D、Hcy、hs-CRP水平均高于中度组及轻度组,中度组血清D-D、Hcy、hs-CRP水平均高于轻度组,差异均有统计学意义(P<0.05)。经过Spearman相关性分析,D-D、Hcy、hs-CRP水平与患者梗死灶面积均呈正相关(r=0.694、0.659、0.705,P<0.05),与神经缺损程度均呈正相关(r=0.676、0.681、0.711,P<0.05)。结论:急性脑梗死患者体内血清D-D、Hcy、hs-CRP水平均显著升高,且其水平与梗死面积、神经缺损程度有关,因此血清D-D、Hcy、hs-CRP水平可作为急性脑梗死患者的重要检测指标和治疗靶点。 Objective:To study the changes and significance of D-dimer (D-D),homocysteine (Hcy),high sensitivity C-reactive protein (hs-CRP) levels in patients with acute cerebral infarction.Method:A total of 200 patients with acute cerebral infarction admitted to our hospital from February 2018 to July 2020 were selected as the observation group.In addition,100 healthy volunteers without related cerebrovascular diseases were selected as the control group.The levels of serum D-D,Hcy and hs-CRP were compared between the two groups.The levels of serum D-D,Hcy and hs-CRP in patients with cerebral infarction with different infarct diameters and different degrees of nerve defects in the observation group were compared,and Spearman correlation analysis was used to analyze the correlation between the levels of D-D,Hcy and hs-CRP and infarct size and degree of nerve defects.Result:The serum levels of D-D,Hcy and hs-CRP in the observation group were all higher than those in the control group,the differences were statistically significant (P<0.05).The serum levels of D-D,Hcy and hs-CRP in the large infarction group were all higher than those in the medium infarction group and the small infarction group,and the levels of D-D,Hcy and hs-CRP in the medium infarction group were all higher than those in the small infarction group,the differences were statistically significant (P<0.05).The levels of D-D,Hcy and hs-CRP in the severe group were all higher than those in the moderate and mild groups,and the levels of D-D,Hcy and hs-CRP in the moderate group were all higher than those in the mild group,the differences were statistically significant (P<0.05).According to Spearman correlation analysis,the levels of D-D,Hcy and hs-CRP were positively correlated with infarct size (r=0.694,0.659,0.705,P<0.05) and nerve defect degree (r=0.676,0.681,0.711,P<0.05).Conclusion:Serum D-D,Hcy,hs-CRP levels in patients with acute cerebral infarction are significantly increased,and their levels are related to infarct size and nerve defect degree.Therefore,serum D-D,Hcy,hs-CRP levels can be used as important detection indicators and therapeutic targets for patients with acute cerebral infarction.
