髋臼前柱螺钉通道的解剖学参数

Anatomical parameters of anterior column screw tunnels of the acetabulum

目的研究髋臼前柱螺钉的可用直径、长度及髋臼前柱截骨平面同心圆直径参数。方法将59例健康者薄层骨盆CT数据以DICOM格式导入Mimics软件建立模型,测量前柱螺钉的直径、长度及前柱截骨平面同心圆直径。结果前柱螺钉男性可用直径为6.1~7.8(6.9±0.4)mm,可用长度为96.7~112.1(105.2±3.7)mm;女性可用直径为4.9~6.6(5.9±0.4)mm,可用长度为91.3~103.0(95.7±3.0)mm;男、女可用螺钉直径及长度比较差异有统计学意义(P<0.001)。同一性别左、右侧可用螺钉直径及长度比较差异均无统计学意义(P>0.05)。髋臼前柱第5、10截骨平面为髋臼前柱狭窄处,第5截骨平面同心圆直径男性为10.3~12.8(11.3±0.7)mm,女性为6.8~9.4(8.0±0.7)mm,第10截骨平面同心圆直径男性为9.2~11.0(10.1±0.5)mm,女性为6.2~8.5(7.3±0.6)mm,男、女可用螺钉直径与第5、10截骨平面同心圆直径比较差异有统计学意义(P<0.001)。结论男、女髋臼前柱螺钉参数存在差异,实际应用中不能以狭窄处截骨平面同心圆直径为参照去选择前柱螺钉的直径。

Objective To study the parameters of useable diameter and length of the acetabular anterior column screw,and concentric circles diameters of osteotomy plane of the acetabular anterior column.Methods The thin-layer pelvic CT data of 59 healthy people were imported into Mimics software with DICOM format to establish the model.The following data were measured,including the diameter and length of the anterior column screw,concentric circles diameters of osteotomy plane of the anterior column.Results The useable diameter and length of anterior column screw:they were 6.1~7.8(6.9±0.4)mm and 96.7~112.1(105.2±3.7)mm in the males;and 4.9~6.6(5.9±0.4)mm,91.3~103.0(95.7±3.0)mm in the females.There were statistical differences for the useable diameter and length in the males and females(P<0.001).For the same sex people,the useable diameter and length of left and right sides had no statistical significance(P>0.05).The 5th and 10th osteotomy planes of the acetabular column were the narrow site of the acetabular anterior column.For the diameters of concentric circles on the 5th osteotomy planes,the range of males was 10.3~12.8(11.3±0.7)mm,and the range of females was 6.8~9.4(8.0±0.7)mm.For the diameters of concentric circles on the 10th osteotomy planes,the range of males was 9.2~11.0(10.1±0.5)mm,the range of females was 6.2~8.5(7.3±0.6)mm.Compared with the useable screws diameters of the males and females,there were differences with the diameters of concentric circles on the 5th,10th osteotomy planes(P<0.001).Conclusions There are differences for the parameters of acetabular anterior column screws between the males and females.The diameter of the anterior column screw can′t be selected by reference of the diameter of the concentric circle of the osteotomy plane at the narrow site in the practice.
