股骨近端防旋髓内钉治疗老年A2型股骨转子间骨折

The proximal femoral nail antirotation in the treatment of elderly type A2 intertrochanteric fracture

目的探讨股骨近端防旋髓内钉(PFNA)治疗老年A2型股骨转子间骨折的近期疗效。方法采用PFNA治疗41例老年A2型股骨转子间骨折患者。记录手术时间、术中出血量、术中透视次数。末次随访时采用Harris评分评价髋关节功能。结果手术时间20~90(55.6±21.7)min,术中出血量100~400(200.4±80.5)ml,术中透视次数22~36(29.5±6.4)次。患者均获得随访,时间3~12个月。末次随访时采用Harris评分评价髋关节功能:优32例,良6例,可2例,差1例,优良率92.68%。结论PFNA治疗老年A2型股骨转子间骨折具有创伤小、手术时间短、术中出血量少、髋关节功能恢复好等优点,临床疗效肯定。

Objective To investigate the short-term effect of proximal femoral nail antirotation(PFNA)internal fixation in the treatment of elderly type A2 intertrochanteric fracture.Methods The 41 elderly patients with type A2 intertrochanteric fracture were treated with PFNA.The operation time,intraoperative bleeding volume,intraoperative fluoroscopy times were recorded.Harris score was used to evaluate hip function at the last follow-up.Results The operation time was 20~90(55.6±21.7)min,the intraoperative bleeding volume was 100~400(200.4±80.5)ml,and the fluoroscopy times was 22~36(29.5±6.4)times.All patients were followed up from 3 months to 12 months.At the last follow-up,the curative effect was evaluated according to Harris functional score of hip joint:excellent in 32 cases,good in 6,fair in 2,poor in 1,the excellent and good rate was 92.68%.Conclusions PFNA for treatment of elderly type A2 intertrochanteric fracture of femur is less trauma,shorter operation time,less bleeding,good recovery of hip function and remarkable curative effect.
