跗骨窦切口治疗SandersⅡ、Ⅲ型跟骨骨折

Treatment of Sanders typeⅡandⅢcalcaneal fractures with tarsal sinus incision

目的探讨跗骨窦切口治疗SandersⅡ、Ⅲ型跟骨骨折的临床疗效。方法采用跗骨窦切口治疗30例SandersⅡ、Ⅲ型跟骨骨折患者。记录并发症情况,测量跟骨Böhler角、Gissane角、跟骨宽度及高度,根据AOFAS踝-后足评分系统评价疗效。结果患者均获得随访,时间12~18个月。Gissane角、Böhler角、跟骨宽度及高度术后3、6个月均较术前改善(P<0.05)。骨折均愈合,未发生切口皮肤坏死、深部感染、内固定位置改变和骨折复位丢失。末次随访根据AOFAS踝-后足评分系统评价疗效:优20例,良6例,可4例,优良率26/30。结论采用跗骨窦切口治疗跟骨骨折具有创伤小、并发症少等优点,疗效满意。

Objective To investigate the clinical efficacy of tarsal sinus incision in the treatment of Sanders typeⅡandⅢcalcaneal fractures.Methods Thirty cases of Sanders typeⅡandⅢcalcaneal fractures were treated with tarsal sinus incision.The complications were recorded,the calcaneal Böhler angle,Gissane angle,calcaneal width and height were measured,and the curative effect was evaluated according to the AOFAS ankle-hindfoot function score criterion.Results All patients were followed up for 12~18 months.At 3,6 months after operation,Gissane angle,Böhler angle,calcaneal width and height were improved than the preoperation(P<0.05).All fractures healed without incisal edge skin necrosis,deep infection,change of internal fixation position and loss of fracture reduction.At the last follow-up,the curative effect was evaluated according to AOFAS ankle-hindfoot function scoring system:excellent in 20 cases,good in 6,fair in 4,with the excellent-good rate of 26/30.Conclusions The treatment of calcaneus fracture with tarsal sinus incision has the advantages of less trauma,few complications,with satisfactory effect.
