基于个案分析的优质护理服务在血液净化患者中的应用

Application of High Quality Nursing Service Based on Case Analysis in Patients with Blood Purification

目的:探讨基于个案分析的优质护理服务在血液净化患者中的应用效果。方法:选取2017年5月1日~2019年5月1日收治的120例血液净化患者作为研究对象,依据入院时间顺序分为对照组和观察组各60例。对照组给予常规护理干预,观察组在常规护理干预基础上施行基于个案分析的优质护理服务。两组均进行5个月护理干预,统计两组干预前后情绪变化[采用正负性情绪状态量表(PANAS)]、应对方式[采用医学应对方式问卷(MCMQ)]、血液净化相关知识掌握度、自我管理能力、护理满意度[采用纽卡斯尔护理服务满意度量表(NSNS)],比较两组并发症发生情况。结果:干预5个月后,两组PANAS中PA得分高于干预前(P<0.05)、NA得分低于干预前(P<0.05),且观察组优于对照组(P<0.01);干预5个月后,两组MCMQ中屈服、回避得分低与干预前(P<0.05)、面对得分高于干预前(P<0.05),且观察组优于对照组(P<0.01);观察组血液净化相关知识掌握度高于对照组(P<0.01,P<0.05),并发症发生率低于对照组(P<0.05);干预5个月后,两组遵医用药、操作技术、病情监测、饮食摄入、并发症管理得分高于干预前(P<0.05),且观察组高于对照组(P<0.01);观察组护理满意度高于对照组(P<0.01)。结论:基于个案分析的优质护理服务应用于血液净化患者中,能改善患者情绪,提高疾病认知,增强自我管理能力,积极应对治疗,对减少并发症、提升护理满意度有积极意义。

Objective:To explore the application effect of high quality nursing service based on case analysis in patients with blood purification.Methods:A total of 120 patients with blood purification admitted from May 1,2017 to May 1,2019 were selected as study subjects,and were divided into control group and observation group with 60 patients each according to the time sequence of admission.The control group received routine nursing intervention,and the observation group implemented high quality nursing service based on case analysis on the basis of routine nursing intervention.Two groups had received nursing intervention for five months.The mood changes[using positive and negative affect scale(PANAS)],coping styles[using medical coping modes questionnaire(MCMQ)],knowledge related blood purification to master degree,self-management ability,and nursing satisfaction[the Newcastle satisfaction with nursing scale(NSNS)]of two groups were collected before and after the intervention.The occurrence of complications was compared between the two groups.Results:After 5 months of intervention,PA score in PANAS of the two groups was higher than that before intervention(P<0.05),NA score was lower than that before intervention(P<0.05),and the observation group was better than the control group(P<0.01).After 5 months of intervention,the scores of yield and avoidance in MCMQ of the two groups were lower than those before intervention(P<0.05)and face score was higher than before intervention(P<0.05),and the observation group was better than the control group(P<0.01).The knowledge of blood purification in the observation group was higher than that in the control group(P<0.01,P<0.05),and the incidence of complications was lower than that in the control group(P<0.05).After 5 months of intervention,the scores of compliance with medical drugs,operation techniques,disease monitoring,dietary intake and complication management in the two groups were higher than those before intervention(P<0.05),and those in the observation group were higher than those in the control group(P<0.01).Nursing satisfaction in the observation group was higher than that in the control group(P<0.01).Conclusion:The application of high quality nursing service based on case analysis in patients with blood purification can improve patients′mood,promote disease cognition,enhance self-management ability,and actively respond to treatment.It is of positive significance to reduce complications and improve nursing satisfaction.
