肢体气压治疗联合综合护理模式对脑梗死偏瘫患者DVT的影响

Influence of Limb Pressure Therapy Combined with Comprehensive Nursing on DVT in Patients with Cerebral Infarction Hemiplegia

目的:探讨肢体气压治疗联合综合护理对脑梗死偏瘫患者深静脉血栓形成(DVT)的影响。方法:将2018年6月1日~2020年1月1日就诊的90例脑梗死偏瘫患者,依据护理方法不同分为观察组和对照组各45例,对照组实施综合护理模式,观察组在对照组基础上实施肢体气压治疗;比较两组护理前后血流峰速度、平均速度、DVT发生率、心理状态[采用焦虑自评量表(SAS)、抑郁自评量表(SDS)]、运动功能[采用简化Fugl-Meyer评定法]和生活能力[采用改良Barthel指数]。结果:护理后,观察组血流峰速度、平均速度、Fugl-Meyer评分及Barthel指数均优于对照组(P<0.01);观察组DVT发生率低于对照组(P<0.05);护理后,两组SAS、SDS评分均低于护理前(P<0.05,P<0.01),且观察组低于对照组(P<0.05,P<0.01)。结论:对脑梗死偏瘫患者实施肢体气压治疗联合综合护理模式,能缓解患者不良情绪,有效防止DVT,促进下肢静脉回流,从而提高患者运动能力和生活自理能力。

Objective:To explore the effect of limb pressure therapy combined with comprehensive nursing on deep vein thrombosis(DVT)in patients with cerebral infarction hemiplegia.Methods:A total of 90 patients with cerebral infarction hemiplegia who visited the hospital from June 1,2018 to January 1,2020 were divided into the observation group and the control group with 45 cases each according to different nursing methods.The control group received comprehensive nursing,and the observation group received limb pressure treatment on this basis.Peak velocity of blood flow,average velocity,incidence of DVT,psychological status(SAS and SDS),motor function(simplified Fugl-Meyer assessment)and life ability(improved Barthel index)were compared between the two groups before and after nursing.Results:After nursing,the observation group of peak velocity,average velocity,Fugl-Meyer score and Barthel index were all better than the control group(P<0.01).The incidence of DVT in the observation group was lower than that in the control group(P<0.05).After nursing,SAS and SDS scores of the two groups were lower than before nursing(P<0.05,P<0.01),and the observation group was lower than the control group(P<0.05,P<0.01).Conclusion:The combination of limb pressure therapy and comprehensive nursing can relieve patients′bad mood,effectively prevent DVT and promote venous reflux of lower limbs,so as to improve patients′motor ability and self-care ability.
