摘要
目的探究静脉镇痛泵和股神经阻滞在膝关节置换术后应用的疗效。方法以2019年1月—2020年1月收治的膝关节置换术患者80例为对象,分为参照组40例与研究组40例,参照组应用静脉镇痛泵,研究组实施股神经阻滞,对比镇痛效果、不良反应、地佐辛注射次数、按压自控键次数,进行对比分析。结果研究组术后患者静息状态下疼痛程度评分术后12 h(4.21±1.09)分、术后1 d(3.32±0.87)分、术后3 d(2.07±0.69)分、术后7 d(1.43±0.54)分均优于参照组的(4.97±0.97)分、(4.07±0.96)分、(3.08±0.74)分、(2.68±0.64)分,研究组术后患者活动状态下疼痛程度评分术后3 d(2.78±0.58)分、术后7 d(1.87±0.79)分均优于参照组的(3.24±0.63)分、(2.58±0.68)分,研究组地佐辛注射次数(3.33±0.95)次、按压自控键次数(9.09±2.54)次低于参照组的(5.09±0.86)、(13.78±2.48)次,差异有统计学意义(t=4.098、3.979、4.734、5.097,4.238、4.968,4.348、5.068,P<0.05);研究组患者麻醉药物不良反应发生率5%低于参照组20%,差异有统计学意义(χ^2=4.114,P<0.05)。结论膝关节置换术患者术后使用股神经阻滞镇痛,有效缓解术后疼痛,降低不良反应发生率,减少地佐辛使用剂量,镇痛效果显著。
Objective To explore the effect of intravenous analgesia pump and femoral nerve block after knee arthroplasty.Methods From January 2019 to January 2020,80 patients with knee arthroplasty were selected as the subjects.They were divided into a reference group of 40 cases and a study group of 40 cases.The reference group was treated with intravenous analgesia pump,and the study group was implemented nerve block,compare the analgesic effects,adverse reactions,the number of injections of dezocine,and the number of times the automatic control key pressed for comparative analysis.Results The postoperative pain level score of the patients in the study group was at rest for 12 hours after the operation(4.21±1.09)points,1 day after the operation(3.32±0.87)points,3 days after the operation(2.07±0.69)points,and 7 days after the operation(1.43±0.54)points.In the reference group(4.97±0.97)points,(4.07±0.96)points,(3.08±0.74)points,(2.68±0.64)points,the pain level score of the patients in the study group after the operation was 3 days(2.78±0.58)points after the operation,and 7 days after the operation(1.87±0.79)points were better than the reference group's(3.24±0.63)points,(2.58±0.68)points,the study group's dezocine injection times were(3.33±0.95)times,and the number of automatic control keys pressed(9.09±2.54)times were lower than the reference group's(5.09±0.86)times,(13.78±2.48)times,the difference was statistically significant(t=4.098,3.979,4.734,5.097,4.238,4.968,4.348,5.068,P<0.05);the incidence of adverse reactions of anesthetics in the study group was 5% lower than 20% in the reference group,the difference was statistically significant(χ^2=4.114,P<0.05).Conclusion The use of femoral nerve block analgesia after knee arthroplasty can effectively relieve postoperative pain,reduce the incidence of toxic and side effects,and reduce the dosage of dezocine.The analgesic effect is significant.
