血清SAA、全程CRP检测诊断早产儿感染性疾病的效果 被引量:1

Effect of Serum SAA and CRP Detection in the Diagnosis of Infectious Diseases in Premature Infants

摘要 目的评价血清SAA、全程CRP检测诊断早产儿感染性疾病的效果。方法对该院2018年2月—2019年11月新生儿病房收治的早产儿进行回顾性分析,选择96例早产儿作为研究的对象,根据病因分为A组(细菌感染26例)、B组(病毒感染20例)与C组(未感染50例)。3组早产儿均接受了常规治疗,3组早产儿分别接受常规全程CRP检测以及血清SAA、全程CRP联合检测。观察A、B两组治疗前后血清SAA、全程CRP水平,比较两种检查方式检查准确率、灵敏度与特异性。结果治疗前,A组SAA、全程CRP水平明显高于B组;治疗后,两组SAA、全程CRP水平均显著降低,但A组SAA(9.72±3.51)mg/L、全程CRP水平(8.92±0.53)mg/L明显高于B组(9.24±3.24)、(2.78±0.62)mg/L,差异有统计学意义(t=36.179、3.618,P<0.05);血清SAA、全程CRP联合检测的准确率92.70%、灵敏度95.65%、特异性97.83%均高于常规全程CRP检测的80.21%、82.61%、84.78%,差异有统计学意义(χ^2=5.206、5.268、5.351,P<0.05)。结论对早产儿采用血清SAA、全程CRP检测能够早期诊断早产儿感染性疾病,可以有效区分感染类型,且血清SAA、全程CRP联合检测准确率、灵敏度以及特异性较高。 Objective To evaluate the efficacy of serum SAA and CRP detection in the diagnosis of infectious diseases in preterm infants.Methods A retrospective analysis of the premature infants admitted to the neonatal ward of the hospital from February 2018 to November 2019 was conducted.96 premature infants were selected as the subjects of this study.According to the cause,they were divided into group A(26 cases of bacterial infection),group B(20 cases with virus infection)and group C(50 cases without infection).The three groups of premature infants all received conventional treatment,and the three groups of premature infants received routine full-course CRP testing and combined detection of serum SAA and full-course CRP.Observe the serum SAA and CRP levels before and after treatment in groups A and B,and compare the accuracy,sensitivity and specificity of the two inspection methods.Results Before treatment,the levels of SAA and full-course CRP in group A were significantly higher than those in group B;after treatment,the levels of SAA and full-course CRP in both groups were significantly reduced,but group A had SAA(9.72±3.51)mg/L and full-course CRP levels(8.92±0.53)mg/L was significantly higher than(9.24±3.24)mg/L,(2.78±0.62)mg/L in group B,with statistical differences(t=36.179,3.618,P<0.05);the accuracy of combined detection of serum SAA and CRP was 92.70%,the sensitivity of 95.65% and specificity of 97.83% were higher than those of 80.21%,82.61%,and 84.78% of the routine CRP detection.The difference was statistically significant(χ^2=5.206,5.268,5.351,P<0.05).Conclusion The use of serum SAA and full-course CRP detection for preterm infants can early diagnose infectious diseases in premature infants,and can effectively distinguish the types of infections.The combined detection of serum SAA and full-course CRP has high accuracy,sensitivity and specificity and is worthy of application.
