严重肺部感染机械通气患者不同时期血气分析采集效果探讨

摘要 目的探析严重肺部感染机械通气患者不同时期血气分析采集效果。方法选取36例2017年4月—2020年2月在该院治疗的严重肺部感染机械通气患者为观察对象,采用随机分组法分成对照组与实验组,每组18例。对照组患者遵照医嘱予以常规吸痰,吸痰后马上采集血气标本,实验组患者遵照医嘱予以改进吸痰,于吸痰结束后不同时期采集血气标本,比较两组患者吸痰结束时、吸痰后5 min血气指标水平以及临床各项指标变化。结果在吸痰结束时,对照组患者PaO2(血氧分压)、SaO2(血氧饱和度)分别为(12.83±1.21)kPa、(93.18±1.65)%,实验组患者分别为(16.03±1.34)kPa、(96.17±1.73)%,比较差异有统计学意义(t=6.250、7.610,P<0.05);在吸痰后5 min,对照组患者分别为(12.33±1.13)kPa、(94.23±1.71)%,实验组患者分别为(16.98±1.43)kPa、(98.01±1.37)%,组间差异有统计学意义(t=6.430、7.280,P<0.05)。对照组患者吸痰后心率、血压分别为(102.65±10.25)次/min、(115.37±13.27)mmHg,实验组患者分别为(94.37±9.27)次/min、(98.86±11.38)mmHg,两组吸痰后心率、血压均明显降低,且实验组明显低于对照组,差异有统计学意义(t=8.670、8.730,P<0.05)。结论在严重肺部感染患者机械通气治疗中,选择恰当的时机采集血气标本,有助于预防患者机体缺氧,提高患者血氧含量,值得临床深入研究。 Objective To explore the effect of blood gas analysis in different periods of mechanically ventilated patients with severe lung infection.Methods Thirty-six mechanically ventilated patients with severe lung infection treated in the hospital from April 2017 to February 2020 were selected as observation objects.They were randomly divided into control group and experimental group,with 18 cases in each group.Patients in the control group were routinely suctioned according to the doctor's instructions,and blood gas samples were collected immediately after suction.The patients in the experimental group were treated with improved suction according to the doctor's instructions,and blood gas samples were collected at different periods after the completion of suction.The blood gas index level and the changes of various clinical indexes after 5 min.Results At the end of sputum suction,the PaO2(partial pressure of blood oxygen)and SaO2(blood oxygen saturation)of the control group were(12.83±1.21)kPa and(93.18±1.65)%,respectively,and the experimental group were(16.03±1.34)kPa,(96.17±1.73)%,the difference was statistically significant(t=6.250,7.610,P<0.05);5 min after sputum suction,the control group patients were(12.33±1.13)kPa,(94.23±1.71)%,the patients in the experimental group were(16.98±1.43)kPa and(98.01±1.37)% respectively,with statistically significant differences between the two groups(t=6.430,7.280,P<0.05).The heart rate and blood pressure of the patients in the control group were(102.65±10.25)beats/min and(115.37±13.27)mmHg after sputum suction,and the patients in the experimental group were(94.37±9.27)beats/min and(98.86±11.38)mmHg,respectively.Heart rate and blood pressure were significantly reduced after sputum suction,and the experimental group was significantly lower than the control group,the difference was statistically significant(t=8.670,8.730,P<0.05).Conclusion In the treatment of patients with severe pulmonary infection with mechanical ventilation,choosing the right time to collect blood gas samples can help prevent the patient's body from hypoxia and increase the patient's blood oxygen content.It is worthy of in-depth clinical research.
