临床药师参与1例进展性脑梗死并抑郁障碍患者的药学监护

Clinical Pharmacist’s Participation in Pharmaceutical Care of a Patient with Progressive Cerebral Infarction Complicate with Depression

目的探讨临床药师在进展性脑梗死并抑郁障碍治疗过程中的作用。方法针对医院收治的1例进展性脑梗死并抑郁障碍患者,临床药师结合患者年龄、疾病、用药史、药物相互作用及药品不良反应等情况进行药学监护,包括参与治疗方案的制订,指导护士规范用药,对患者进行用药教育,促进合理用药。结果患者临床症状明显好转,病情稳定,出院后进行康复训练。结论临床药师可更好地利用自身药学专业知识积极参与临床药物治疗方案的制订,协助医师优化临床给药方案。

Objective To investigate the role of clinical pharmacists in the treatment of progressive cerebral infarction complicated with depression.Methods For a patient with progressive cerebral infarction complicated with depression in our hospital,clinical pharmacists provided pharmaceutical care based on the patient’s age,disease,medication history,drug interaction and adverse drug reactions,including participating in the formulation of the treatment plan,guiding nurses to standardize medication,conducting medication education for patients,and promoting rational drug use.Results The patient’s clinical symptoms were obviously improved,and his condition was stable,and the rehabilitation training was carried out after discharge.Conclusion Clinical pharmacists can make better use of their own pharmaceutical professional knowledge,actively participate in the formulation of clinical treatment plan,and assist physician in optimizing the clinical medication plan.
