摘要
目的探讨食管癌同步放化疗患者加强营养管理,对患者营养状况、不良反应发生以及生活质量的影响。方法选取2018年5月—2019年5月南阳市中心医院收治的食管癌同步放化疗患者130例为研究对象,依据基线资料可比的原则分为对照组和观察组,每组65例。对照组行常规护理干预,观察组在对照组基础上给予精细化营养管理干预。比较两组患者营养状况、不良反应发生以及生活质量的变化。结果干预前两组患者营养状况各指标比较,差异无统计学意义(P>0.05)。干预后,观察组患者营养状况各指标优于对照组,差异有统计学意义(P<0.05)。观察组患者放射性食管炎、感染发生率明显低于对照组,差异有统计学意义(P<0.05)。两组患者骨髓抑制、放射性肺炎的发生率比较,差异无统计学意义(P>0.05)。干预前两组患者肿瘤患者生活质量自评量表(EORTC QLQ-C30)评分比较,差异无统计学意义(P>0.05)。干预后,观察组患者肿瘤患者EORTC QLQ-C30评分优于对照组,差异有统计学意义(P<0.05)。结论食管癌同步放化疗患者加强营养管理,可以降低治疗不良反应发生率,提高患者营养状况以及生活质量。
Objective To explore the effects of strengthening nutritional management in patients undergoing concurrent radiotherapy and chemotherapy for patients with esophageal cancer on the nutritional status,occurrence of adverse reactions,and quality of life.Methods A total of 130 patients with esophageal cancer who underwent concurrent radiotherapy and chemotherapy and were admitted to Nanyang Central Hospital from May 2018 to May 2019 were selected as the study objects.Based on the principle of comparable baseline data,they were divided into a control group and an observation group,with 65 cases in each group.The control group received routine nursing intervention,and the observation group was given refined nutrition management intervention on the basis of the control group to compare the nutritional status,occurrence of adverse reactions and changes in the quality of life of the two groups.Results Before the intervention,there was no statistically significant difference in the nutritional status of the two groups of patients(P>0.05).After the intervention,the nutritional status indicators of the observation group were better than those of the control group,and the difference was statistically significant(P<0.05).The incidence of radiation esophagitis and infection in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of bone marrow suppression,radiation pneumonitis between the two groups(P>0.05).Before the intervention,there was no statistically significant difference between the two groups of patients on the self-rating quality of life scale(EORTC QLQ-C30)score comparison of cancer patients(P>0.05).After the intervention,the EORTC QLQ-C30 scores of tumor patients in the observation group were better than those in the control group,and the difference was statistically significant(P<0.05).Conclusion Enhancing nutritional management for patients with esophageal cancer undergoing concurrent radiotherapy and chemotherapy can reduce the incidence of adverse treatment reactions and improve the nutritional status and living quality of patients.
