摘要
目的旨在探讨PD-L1表达与CD8阳性T细胞在乳腺癌中的相关性,分析PD-L1与乳腺癌分子分型及预后的关系。方法采用免疫组织化学方法检测潍坊市人民医院病理科144例乳腺癌术后石蜡样本中PD-L1表达水平、CD8阳性T细胞数量,结合ER、PR、Ki-67、HER-2表达及p53蛋白表达情况,分析PD-L1、CD8阳性T细胞、分子分型及患者临床资料相关性。结果22.9%乳腺癌标本中PD-L1高表达(33/144),15.3%乳腺癌标本中CD8阳性T细胞增多(22/144),二者比较有统计学意义(P=0.01);PD-L1、p53的表达在乳腺癌四种分子分型中存在差异(P<0.01),且与四种分子分型的恶性程度相关(P<0.01);绝经后患者肿瘤组织中PD-L1表达明显增高(P=0.03);Ki-67指数30%以上患者PD-L1表达增高(P=0.02)。结论PD-L1表达与CD8阳性T细胞数量显著正相关;PD-L1表达水平与乳腺癌分子亚型相关;PD-L1的表达水平可为作为检测指标为乳腺癌患者的预后评估提供参考。
Objective To investigate the correlation between PD-L1 expression and CD8 positive T cells in breast cancer,and to analyze the relationship between PD-L1 and breast cancer molecular typing and prognosis.Methods Immunohistochemical method was used to detect the expression level of PD-L1 and the number of CD8 positive T cells in 144 cases of breast cancer with postoperative paraffin samples in the Department of Pathology of Weifang People's Hospital,combined with the expression of ER,PR,Ki-67,HER-2 and p53 protein expression status,analysis of PD-L1,CD8 positive T cells,molecular typing and correlation of patients’clinical data.Results PD-L1 was highly expressed in 22.9%of breast cancer specimens(33/144),and CD8-positive T cells increased in 15.3%of breast cancer specimens(22/144),which was statistically significant(P=0.01).The expression of PD-L1 and p53 was different among the four molecular types of breast cancer(P<0.01),but related to the malignant degree of molecular typing(P<0.01);the expression of PD-L1 in tumor tissues of postmenopausal patients was significantly increased(P=0.03);PD-L1 expression increased in patients with Ki-67 index above 30%(P=0.02).Conclusion PD-L1 expression is significantly positively correlated with the number of CD8 positive T cells;PD-L1 expression level is correlated with breast cancer molecular subtypes.Therefore,PD-L1 expression level can be used as a detection index for prognostic evaluation of breast cancer patients.
