摘要
研究了支持多方参与慢病管理模式的信息化平台。基于微信平台,采用微服务分布式技术架构,建立开放式医疗服务平台。移动端对接微信服务号作为数据采集端使用,PC端使用B/S应用进行数据管理。实现了三部分功能:慢病患者的线上自主管理和健康管理信息共享;多学科健康管理团队参与慢病持续管理;挖掘健康管理数据在科研方面的应用。基于微信平台开发互联网慢病管理应用,符合慢病管理的特点,可以满足医患共同健康管理需要,提升健康服务水平。
This paper explores and practices an information platform which supports a chronic disease management model participated by multiple parties.Based on WeChat,the microservice distributed architecture is adopted to build an open medical service platform.The mobile terminal uses the WeChat service number as the data acquisition terminal,while the PC uses the B/S application for data management.Three functions are realized:online self management and health management information sharing of chronic disease patients;management;application of mining health management data in scientific research.The development of Internet chronic disease management application based on WeChat platform is in line with the characteristics of chronic disease management,can meet the needs of medical and patient common health management,and improve the level of health services.
