充气式保温毯对经尿道前列腺电切术后老年患者体温及热舒适度的影响

Effect of forced-air warming blanket on body temperature and thermal comfort in elderly patients after transurethral resection of the prostate

目的探讨充气式保温毯对经尿道前列腺电切术后麻醉恢复期的老年患者体温和热舒适度的影响。方法筛选拟择期行经尿道前列腺电切术的老年患者68例,随机分为常规护理组和保温组各34例,常规护理组在进入麻醉恢复室后加盖普通棉被,保温组采用充气式保温毯进行保温。记录患者术后0 min、10 min、20 min、40 min、60 min的体温,低体温和寒战的发生情况、热舒适度和护理满意度。结果重复测量的方差分析结果显示,术后不同时间点的患者体温的差异有统计学意义(P<0.01);不同分组的患者体温的差异有统计学意义(P<0.001),保温组的体温高于常规护理组;时间和分组的交互效应有统计学意义(P<0.001),两组患者的体温在不同时间点的变化趋势不同,常规护理组患者的体温逐渐降低,保温组患者的体温逐渐升高。保温组的低体温发生率(17.6%)低于常规护理组(44.1%);保温组的寒战发生率(14.7%)低于常规护理组(41.2%);保温组的热舒适度评分(7.88±0.81分)高于常规护理组(7.29±0.97分);保温组的护理满意度评分(4.24±0.64分)高于常规护理组(3.54±0.65分)(均P<0.05)。结论充气式保温毯对TURP术后老年患者具有良好的保温效果,能降低寒战发生率,提供更好的热舒适度体验,患者满意度更高。

Objective To explore the effect of forced-air warming blanket on body temperature and thermal comfort in elderly patients undergoing anesthesia recovery after transurethral resection of prostate(TURP).Methods 68 elderly patients undergoing TURP were randomly divided into the routine care group and the warming-up group,with 34 cases in each group.The routine care group were covered with an ordinary quilt after entering the anesthesia recovery room,while the warming-up group were covered with a forced-air warming blanket.The body temperature of the patients at 0 min,10 min,20 min,40 min after surgery was recorded,as well as the hypothermia and chills,thermal comfort and nursing satisfaction during resuscitation.Results The analysis of repeated measures at different time points showed that the body temperature changed after surgery(P<0.01),and the difference in body temperature between the two groups was statistically significant(P<0.001).The body temperature of the warming-up group was higher than that of routine care group.Moreover,the interaction between time and group was statistically significant(P<0.001).The body temperature of the patients in the routine care group gradually decreased,while the body temperature of the patients in the warming-up group gradually increased.The incidence of hypothermia or chills in the warming-up group(17.6% and 14.7%)was lower than that in the routine care group(44.1% and 41.2%).The thermal comfort score in the warming-up group was(7.88±0.81)was higher than the routine care group(7.29±0.97).The nursing satisfaction score of the warming group(4.24±0.64)was higher than that of the routine care group(3.54±0.65)(all P<0.05).Concusion The forced-air warming blanket provided a good thermal insulation and thermal comfort experience for elderly patients after TURP.
