摘要 目的:回顾分析宫颈癌患者外周血叉状头螺旋转录因子3(Foxp3)mRNA、鳞状细胞癌抗原(SCCA)等的水平变化,相关性及联合检测对诊断宫颈癌的价值。方法:92例宫颈癌患者(宫颈癌组)和87例体检健康女性(对照组)分别采用RT-PCR和化学发光法检测两组外周血Foxp3 mRNA和SCCA以及糖类抗原CA50、CA125水平;Pearson法分析Foxp3 mRNA与SCCA和CA50、CA125水平的相关性;绘制ROC曲线,分析Foxp3 mRNA联合SCCA检测对诊断宫颈癌的价值。结果:宫颈癌组Foxp3 mRNA、SCCA、CA50和CA125水平明显高于对照组(13.71±5.27 vs 2.09±1.51、13.07±1.82 vs 2.85±0.08ng/ml、49.2±13.29 vs 15.61±6.94IU/ml、51.72±15.26 vs 11.57±7.64U/ml,均P<0.01)。宫颈癌患者Foxp3mRNA相对表达水平与SCCA含量呈正相关(r=0.577,P<0.05),但与CA50和CA125无明显相关性(r=0.013和0.085,均P>0.05)。ROC曲线分析显示,Foxp3 mRNA联合SCCA诊断宫颈癌的曲线下面积(AUC)为0.911(95%CI:0.760—0.995),灵敏度为93.10%,特异度为84.70%,明显高于单检Foxp3mRNA或SCCA(均P<0.05)。结论:Foxp3 mRNA与SCCA联合检测可提高对宫颈癌的诊断效能。 Objective:To review the changes and correlations of Foxp3 mRNA,SCCA,CA50 and CA125 levels in patients with cervical cancer,and to analyze the value of joint detection of Foxp3 mRNA and SCCA to the diagnosis of cervical cancer.Method:The levels of Foxp3 mRNA,SCCA,CA50 and CA125 in 92 patients with cervical cancer(cervical cancer group)and 87 healthy volunteers(control group)were detected by RT-PCR and chemiluminescence respectively.The corrlations of Foxp3 mRNA with SCCA,CA50 and CA125 were analyzed by Pearson method and the diagnostic efficacy of joint detection of Foxp3 mRNA and SCCA for cervical cancer was evaluated by ROC curve.Results:The levels of Foxp3 mRNA,SCCA,CA50 and CA125 in cervical cancer group were significantly higher than those in control group(13.71±5.27 vs 2.09±1.51,13.07±1.82 vs 2.85±0.08ng/ml,49.20±13.29 vs 15.61±6.94IU/ml,51.72±15.26 vs 11.57±7.64U/ml,P<0.05).The relative expression level of Foxp3 mRNA in cervical cancer patients was positively correlated with SCCA(r=0.577,P<0.05),and was wirelessly correlated with CA50 and CA125(r=0.013,0.085,P>0.05).ROC curve analysis showed that the AUC of Foxp3 mRNA combined with SCCA in the diagnosis of cervical cancer was 0.911(95%CI:0.760-0.995),the sensitivity was 93.1%,and the specificity was 84.70%which was significantly highet than that Foxp3 mRNA or SCCA alone(P<0.05).Conclusion:The joint detection of Foxp3 mRNA and SCCA can improve the diagnostic efficiency of cervical cancer.
