健脾化痰活血汤联合左甲状腺素钠片治疗甲状腺功能减退症

Treating hypothyroidism with Jianpi Huatan Huoxue Decoction combined with levothyroxine sodium tablets

目的分析健脾化痰活血汤联合左甲状腺素钠片对甲状腺功能减退症患者血清抗甲状腺球蛋白抗体(TGAb)、甲状腺过氧化物酶抗体(TPOAb)、促甲状腺激素(TSH)水平及脂质代谢的影响。方法选择80例甲状腺功能减退症患者病例,随机分为对照组和观察组,各40例。观察组采用健脾化痰活血汤联合左甲状腺素钠片进行治疗,对照组仅口服左甲状腺素钠片,2组均治疗12周。观察2组治疗12周后的临床疗效,比较2组治疗前和治疗12周后血清TSH、TGAb、TPOAb、血脂水平及动脉硬化情况。结果治疗12周后,对照组治疗总有效率(80.00%,32/40)显著低于观察组(95.00%,38/40)(P<0.05)。与治疗前比较,治疗12周后2组血清TSH、TGAb、TPOAb、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、三酰甘油(TG)水平及颈-足背动脉脉搏波速度(cd-PWV)、颈-股动脉脉搏波速度(cf-PWV)、颈-桡动脉脉搏波速度(cr-PWV)均降低,且观察组低于对照组(P<0.05)。与治疗前比较,治疗12周后2组血清高密度脂蛋白胆固醇(HDL-C)水平升高,观察组高于对照组(P<0.05)。结论采用左甲状腺素钠片联合健脾化痰活血汤治疗甲状腺功能减退症患者,可有效降低患者血清TSH、TGAb、TPOAb水平,改善甲状腺功能,有效改善机体脂质代谢情况,促进动脉硬化好转。

Objective To analyze the effect of Jianpi Huatan Huoxue Decoction combined with levothyroxine sodium tablets on serum Antithyroglobulin Antibodies (TGAb),Thyroid Peroxidase Antibody (TPOAb),Thyroid Stimulating Hormone (TSH) levels and lipid metabolism in patients with hypothyroidism.Methods 80 cases of hypothyroidism patients were selected and randomly divided into control group and observation group,each with 40 cases.The observation group was treated with Jianpi Huatan Huoxue Decoction combined with levothyroxine sodium tablets,and the control group was only treated with levothyroxine sodium tablets.Both groups were treated for 12 weeks.Clinical efficacy of the two groups after 12 weeks of treatment was observed,and serum TSH,TGAb,TPOAb,blood lipid levels and arteriosclerosis of the two groups were compared before and after treatment.Results After 12 weeks of treatment,the total effective rate of the control group was 80.00% (32/40),which was significantly lower than that of the observation group (95.00%,38/40) (P<0.05).Compared with before treatment,serum TSH,TGAb,TPOAb,low density lipoprotein cholesterol (LDL-C),total cholesterol (TC),triglyceride (TG) levels,and carotid-dorsalis arterial pulse wave velocity (cd-PWV),carotid-femoral arterial pulse wave velocity (cf-PWV),carotid-radialarterial pulse wave velocity (cr-PWV) of the two groups decreased after treatment,and all of them were significantly lower in the observation group than in the control group (P<0.05).Compared with before treatment,the serum high-density lipoproteincholesterol (HDL-C) levels of the two groups increased after treatment,and the level was higher in the observation group than that in the control group (P<0.05).Conclusion Treatment of hypothyroidism patients with Jianpi Huatan Huoxue Decoction combined with levothyroxine sodium tablets can effectively reduce serum TSH,TGAb and TPOAb levels,improve thyroid function,improve lipid metabolism,and promote the improvement of arteriosclerosis.
