摘要
目的探讨捏脊联合核心稳定性训练对不随意运动型脑瘫患儿的疗效。方法选择本科2017年6月至2019年6月收治的不随意运动型脑瘫患儿120例作为研究对象,随机分为两组各60例,对照组给予小儿捏脊和常规康复治疗,观察组在对照组基础上增加核心稳定性训练治疗,治疗3个疗程后,比较两组患儿核心肌群肌力、核心肌群表面肌电值、粗大运动功能。结果观察组治疗后核心肌群腹直肌、竖脊肌、臀大肌肌力3~5级的百分比明显高于对照组(P<0.05),治疗后两组患儿中立坐位、仰卧起坐位、后伸位两侧腹直肌和竖脊肌表面肌电值明显高于治疗前(P<0.05),治疗后观察组患儿核心肌群表面肌电值明显高于对照组(P<0.05),治疗后两组患儿粗大运动评分明显优于治疗前(P<0.05),治疗后观察组患儿粗大运动评分明显优于对照组(P<0.05)。结论捏脊联合核心稳定性训练能有效增加不随意运动型脑瘫患儿核心肌群肌力,改善粗大运动功能。
Objective To observe the effect of chiropractic combined with core stability training on children with involuntary exercise cerebral palsy.Methods A total of 120 children with involuntary exercise cerebral palsy were selected as subjects from June 2017 to June 2019 in our department and were divided into two groups randomly,with 60 cases in each group.The control group was given chiropractic and routine rehabilitation treatment.The observation group was given core stability training based on the control group.After 3 courses of treatment,the core muscle strength,surface electromyography value,and gross motor function of the two groups were compared.Results The percentage of rectus abdominis,erector spinae,and gluteus maximus muscle strength with 3~5 grade in the observation group after treatment were significantly higher than those in the control group(P<0.05).After treatment,the emgvalues of the rectus abdominis and erector spinae on both sides in neutral sitting position,supine sitting position,and posterior extension position in both groups were significantly higher than before treatment(P<0.05).After treatment,the electromyography values of the rectus abdominis and erector spinae on both sides in neutral sitting position,supine sitting position,and posterior extension position of the observation group were significantly higher than those in the control group(P<0.05).After treatment,the gross motor scores of the two groups were significantly better than before treatment(P<0.05).After treatment,the gross exercise score of the observation group was significantly better than that of the control group(P<0.05).Conclusion Chiropractic combined with core stability training can effectively increase the core muscle strength of children with involuntary exercise cerebral palsy and improve gross motor function.
