急性脑梗死并发应激性溃疡的相关因素分析

Related factors of acute cerebral infarction complicated with stress ulcer

目的探讨急性脑梗死(ACI)患者并发应激性溃疡的相关因素分析。方法选取2016年11月至2019年5月在郑州人民医院就诊并接受相应治疗的120例ACI患者。总结归纳出导致ACI患者并发应激性溃疡的单因素与独立危险因素。结果并发应激性溃疡患者应激性溃疡低于未并发应激性溃疡患者,差异有统计学意义(P<0.05)。通过对ACI患者并发应激性溃疡的单因素分析,结果显示年龄、ACI类型、ACI严重程度、格拉斯哥昏迷评分(GCS)及肠内营养是导致ACI患者发生并发应激性溃疡的影响因素(P<0.05)。通过对ACI患者并发应激性溃疡进行单因素分析,把年龄、ACI类型、ACI严重程度、GCS评分及肠内营养是导致ACI患者发生并发应激性溃疡的影响因素进行多因素Logistic回归分析,结果显示年龄、ACI类型、ACI严重程度及神经功能缺损程度是导致ACI患者并发应激性溃疡的独立危险因素(P<0.05)。结论应激性溃疡在ACI患者中的并发率相对较高,临床应对其年龄、ACI类型及ACI严重程度与神经功能缺损程度等进行更深入地研究,以制定更为完善、有效的防治方案。

【Objective】To explore the related factors of stress ulcer in patients with acute cerebral infarction.【Methods】From December 2016 to May 2019,120 patients with acute cerebral infarction were randomly selected as the study object.The clinical data of patients were analyzed,and the single and independent risk factors of stress ulcer in patients with this disease were summarized.【Results】According to the results of comprehensive analysis,36(30.00%)of 120 patients with acute cerebral infarction were complicated with stress ulcer.Age,type of acute cerebral infarction,degree of neurological deficit[Glasgow Coma Scale(GCS)score],enteral nutrition were the main single factors of acute cerebral infarction patients complicated with stress ulcer(P<0.05).Age,type of acute cerebral infarction,severity of acute cerebral infarction and neurological deficit were independent risk factors for stress ulcer(P<0.05).【Conclusion】The complication rate of stress ulcer in patients with acute cerebral infarction is relatively high.At the same time,age,the type and severity of acute cerebral infarction and the degree of neurological deficit are the main factors and independent risk factors.Therefore,we should focus on the analysis and discussion of the related factors in various aspects in clinical practice,so as to make a more perfect and effective prevention and treatment plan.
