角膜塑形镜去片裸眼视力低下的影响因素及近视控制效果

Influencing factors of low uncorrected visual acuity after orthokeratology lens removal and myopia control effect

目的分析角膜塑形镜去片裸眼视力低下的影响因素,并探讨角膜塑形镜的近视控制效果。方法回顾性分析2017年1月至2018年12月在我院眼科接受视力矫正治疗的200例青少年近视患者的临床资料,比较治疗前、治疗1年后患者的裸眼视力、角膜屈光度、眼轴长度。根据治疗1年后去片裸眼视力对患者进行分组,分析影响角膜塑形镜去片裸眼视力低下的危险因素。结果治疗1年后,患者的裸眼视力增加,角膜屈光度降低,眼轴长度增长,差异具有统计学意义(P<0.05)。200例近视患者的去片裸眼视力低下发生率为6.50%。单因素分析、多因素Logistic回归分析结果显示,年龄、眼轴长度、屈光度数是影响近视患者配戴角膜塑形镜后去片裸眼视力低下的危险因素,差异具有统计学意义(P<0.05)。结论角膜塑形镜对近视患者的近视控制效果良好,可有效提高视力,但少数患者受年龄、眼轴长度、屈光度数的影响,其去片裸眼视力低下。

Objective To analyze the influencing factors of low uncorrected visual acuity after orthokeratology lens removal,and to explore the myopia control effect of orthokeratology.Methods The clinical data of 200 adolescent patients with myopia who received vision correction treatment in our hospital from January 2017 to December 2018 were retrospectively analyzed.The uncorrected visual acuity,corneal diopter and axial length of the patients before treatment and after 1 year of treatment were compared.According to the uncorrected visual acuity after 1 year of treatment,the patients were grouped.The risk factors affecting low uncorrected visual acuity after orthokeratology lens removal were analyzed.Results After 1 year of treatment,the uncorrected visual acuity increased,corneal diopter decreased and axial length increased,and the differences were statistically significant(P<0.05).The incidence of low uncorrected visual acuity after orthokeratology lens removal was 6.50%in 200 patients with myopia.The univariate analysis and multivariate Logistic regression analysis showed that age,axial length and refractive diopter were the risk factors affecting low uncorrected visual acuity after orthokeratology lens removal,and the differences were statistically significant(P<0.05).Conclusion Orthokeratology has a good myopia control effect and can effectively improve vision in patients with myopia.However,due to the influence of age,axial length and refractive diopter,the uncorrected visual acuity after orthokeratology lens removal of a few patients is low.
