24 h动态心电图心电Lorenz散点图对心律失常的诊断和心率变异性的分析价值

Values of 24 h dynamic electrocardiogram Lorenz scatter plot in diagnosis of arrhythmia and analysis of heart rate variability

目的探讨24 h动态心电图心电Lorenz散点图对心律失常的诊断和心率变异性(HRV)的分析价值。方法选取2018年10月至2020年2月收治的200例心律失常患者作为观察组,以及200例健康体检者作为对照组。两组受检者均进行24 h动态心电图,并分析心电Lorenz散点图情况,比较两组HRV测值及Lorenz散点图形态。结果观察组的DC、SDANN、SDNN、rMSSD均低于对照组(P<0.05)。观察组出现彗星状、复杂状的比例高于对照组,出现扇形状的比例低于对照组(P<0.05)。观察组Lorenz散点图为鱼雷状、彗星状、复杂状的患者中,HRV正常范围占比分别为68.75%、91.56%、88.42%。室性并行心律、房性并行心律的B线斜率数值为无穷大,其次为窦性心律。结论24 h动态心电图与各种心律失常散点图形态特征具有较好对应性,而心电Lorenz散点图可反映HRV情况。

Objective To explore the values of 24 h dynamic electrocardiogram Lorenz scatter plot in diagnosis of arrhythmia and analysis of heart rate variability(HRV).Methods A total of 200 patients with arrhythmia admitted from October 2018 to February 2020 were selected as observation group,and 200 healthy physical examination people were selected as control group.The 24 h dynamic electrocardiogram was performed in both groups,and Lorenz scatter plot was analyzed.HRV and Lorenz scatter plot shape were compared between the two groups.Results The DC,SDANN,SDNN and rMSSD of the observation group were lower than those of the control group(P<0.05).The proportions of comet shape and complex shape in the observation group were higher than those in the control group,and the proportion of fan shape was lower than that in the control group(P<0.05).In the observation group,the normal range of HRV accounted for 68.75%,91.56%and 88.42%of the patients with torpedo shape,comet and complex shape of Lorenz scatter plot.The slope of B-line of ventricular parasystole and atrial parallel rhythm was infinite,followed by sinus rhythm.Conclusion The 24 h dynamic electrocardiogram has a good correspondence with the morphological characteristics of various arrhythmia scatter plots,and Lorenz scatter plot can reflect the HRV.
