经阴道病灶切除联合子宫修补术对剖宫产瘢痕妊娠患者术后恢复的影响

Impact of Transvaginal Lesion Resection Combined with Uterine Repair on Postoperative Recovery of Patients with Cesarean Scar Pregnancy

目的探讨经阴道病灶切除联合子宫修补术对剖宫产瘢痕妊娠患者术后恢复的影响。方法回顾性分析2017年1月至2019年12月我院收治的29例剖宫产瘢痕妊娠患者的临床资料,13例采用甲氨蝶呤化疗联合清宫术治疗的患者纳入对照组,16例采用经阴道病灶切除联合子宫修补术治疗的患者纳入观察组。比较两组的相关手术指标、术后恢复及并发症情况。结果观察组的术中出血量低于对照组,住院时间、血清β-hCG转阴时间及月经复潮时间均短于对照组(P<0.05)。两组的手术时间和术后并发症发生率比较,差异无统计学意义(P>0.05)。结论经阴道病灶切除联合子宫修补术治疗剖宫产瘢痕妊娠患者的创伤较小,利于术后恢复,且安全性较高。

Objective To explore the impact of transvaginal lesion resection combined with uterine repair on postoperative recovery of patients with cesarean scar pregnancy.Methods The clinical data of 29 patients with cesarean scar pregnancy admitted to our hospital from January 2017 to December 2019 were retrospectively analyzed.13 patients treated with methotrexate chemotherapy combined with hysterectomy were selected as the control group,and 16 patients treated with transvaginal lesion resection combined with uterine repair were selected as the observation group.The related surgery indicators,postoperative recovery and complications were compared between two groups.Results The amount of intraoperative blood loss of observation group was lower than that of control group,and the length of hospital stay,the time of serumβ-HCG turning negative and the time of menstruation recovery were shorter than those in the control group(P<0.05).No significant difference was found in the operation time and incidence of postoperative complications between two groups(P>0.05).Conclusions Transvaginal lesion resection combined with uterine repair in the treatment of patients with cesarean scar pregnancy has small trauma and high safety,and is conducive to postoperative recovery.
