中国社区老年人脑钠肽水平与衰弱指数的关系 被引量:3

摘要 目的探讨社区老年人脑钠肽(brain natriuretic peptide,BNP)的异常升高与衰弱指数定义衰弱的关系。方法总计1449名年龄在70~89岁的中国社区居民参加了这项横断面研究。选用40个健康指标构建衰弱指数(frailty index,FI)。升高的BNP定义为BNP浓度>100 pg/mL。结果衰弱指数的平均值为0.15±0.09。291名受试者(20.1%)的衰弱指数>0.21。与最低四分位数相比,最高的BNP四分位数具有更高的衰弱风险,多因素调整后的比值比为1.92(95%CI 1.19~3.09)。经协变量调整后,升高的BNP(>100 pg/mL)与衰弱之间关联显著(OR 1.81,95%CI 1.22~2.68)。在多变量调整后,BNP每增加1个标准差,衰弱的风险是1.27倍(95%CI 1.07~1.51)。经过对数转换后的BNP与衰弱指数正相关(r=0.16,P<0.001)。结论血浆BNP的升高与衰弱指数定义的衰弱风险增加相关。 Objective To explore the association of plasma brain natriuretic peptide(BNP)level with frailty index in community-dwelling Chinese elderly.Methods A total of 1449 Chinese community residents aged 7089 years participated in this cross-sectional study.The frailty index(FI)containing 40 health deficits was measured in the study.Elevated plasma BNP level was defined as BNP>100 pg/mL.Results The mean FI value was 0.15±0.09,and 291(20.1%)participants were classified as frail(FI>0.21).Relative to the lowest quartile,the highest BNP quartile had a greater risk of prevalent frailty with adjusted odds ratios of 1.92(95%CI 1.193.09).The elevated BNP level was significantly associated with a higher risk for frailty(OR 1.81,95%CI 1.222.68)after adjustment for covariates.After a multivariable adjustment,the risk of frailty was 1.27(95%CI 1.071.51)for a per-SD increase in BNP.Log BNP was positively correlated with FI(r=0.16,P<0.001).Conclusion Elevated plasma BNP is associated with increased risks of frailty index in Chinese community-dwelling elderly.
