超声引导下干针与体外冲击波治疗在老年足底筋膜炎中的比较

Comparison of ultrasound-guided dry needling and extracorporeal shock wave therapy on plantar fasciitis in the elderly

目的比较超声引导下干针与体外冲击波治疗(ESWT)对老年足底筋膜炎(PF)的临床疗效。方法2018年8月—2020年2月,选取南通市第六人民医院疼痛科收治的58例老年PF患者,随机分为干针组(n=29)和ESWT组(n=29)。分别于治疗前(T_(0))、1个疗程治疗结束后4周(T_(1))、1个疗程治疗结束后12周(T_(2)),对患者的"第一步"-数字疼痛评分(NRS)和改良的Roles and Maudsley评分(RMS)进行比较。结果干针组和ESWT组T_(1)、T_(2)的NRS、RMS均低于同组T0(P<0.01),且各组内T1和T2的NRS、RMS差异均无统计学意义(P>0.05)。干针组T,的NRS、RMS均低于ESWT组(P<0.05),干针组T2的RMS低于ESWT组(P<0.05),2组间T_(2)的NRS差异无统计学意义(P>0.05)。结论超声引导下干针和ESWT对于老年PF均有一定疗效,前者缓解疼痛起效更快,改善足踝功能的效果更佳。

Objective To compare the clinical effect of ultrasound-guided dry needling and extracorporeal shock wave therapy(ESWT)on plantar fasciitis(PF)in the elderly.Methods From August 2018 to February 2020,58 elder patients with PF admitted to the pain department of the Sixth People’s Hospital of Nantong were randomly divided into dry needling group(n=29)and ESWT group(n=29).’First-step’-numerical rating scale(NRS)and modified Roles and Maudsley score(RMS)were compared before treatment(T_(0)),4 weeks after treatment(T_(1))and 12 weeks after treatment(T_(2)).Results NRS and RMS of T_(1)and T_(2)in dry needling group and ESWT group were all lower than T_(0)in the same group(P<0.01),and there was no significant difference in NRS and RMS of T_(1)and T_(2)within each group(P>0.05).The NRS and RMS of T,in the dry needling group were both lower than those in the ESWT group(P<0.05),and the RMS of T_(2)in the dry needling group was lower than that in the ESWT group(P<0.05).There was no significant difference in the NRS of T_(2)between the two groups(P>0.05).Conclusion Ultrasound-guided dry needling therapy and ESWT are both effective for elder patients with PF,and the former can relieve pain faster and improve ankle function better.
