摘要
目的了解农村女性乳腺癌患者就医延迟情况,探讨就医延迟的影响因素。方法采取整群随机抽样的方法,在吉林地区舒兰市、磐石市、桦甸市、蛟河市和永吉县,每个市(县)随机抽取1所二级甲等医院,以在抽取的5所医院就诊并经乳腺科病理确诊的283名术后乳腺癌患者作为研究对象。结果农村乳腺癌患者就医总延迟率为61.5%。多因素Logistic回归分析显示:月收入低(OR:1.539,95%CI:0.669~2.160)、家庭支持得分低(OR:1.350,95%CI:1.119~2.850)、文化程度低(OR:2.651,95%CI:1.920~2.945)是乳腺癌患者就医延迟的影响因素。结论农村乳腺癌患者就医延迟情况严重。月收入低、低文化程度和缺少家庭支持是农村乳腺癌患者就医延迟的主要危险因素。相关部门今后应有针对性地制定干预措施,提高妇女就医意识,降低患病率。
Objective To know about the status quo of patient delay of breast cancer among rural women and to explore the influencing factors of patient delay. Methods A total of 283 postoperative breast cancer patients were selected for the study by cluster random sampling, who were pathologically diagnosed at Department of Breast Surgery of five respective Grade A, Class Two Hospitals in Shulan City, Panshi City, Huadian City, Jiaohe City and Yongji County of Jilin Province.Results The total delay rate in seeking medical advice was 61.5% among rural breast cancer patients. Multivariate Logistic regression analysis showed that low monthly income(OR:1.539, 95%CI: 0.669-2.160), low family support score(OR: 1.350, 95%CI: 1.119-2.850) and low educational level(OR: 2.651, 95%CI: 1.920-2.945) are the influencing factors for the delay of breast cancer patients seeking medical advice. Conclusions The present situation of patient delay is serious among rural breast cancer patients.Low monthly income, low educational level, and lack of family support are the main risk factors for patient delay among rural breast cancer patients. In the future,relevant departments need to formulate targeted interventions to raise women’s awareness of seeking medical advice and reduce the prevalence rate of breast cancer.
