血清脂蛋白相关磷脂酶A2水平与冠状动脉病变及慢血流相关性研究 被引量:10

摘要 目的探讨血清脂蛋白相关磷脂酶A2(LP-PLA2)水平与冠状动脉(冠脉)病变程度及慢血流发生之间的关系。方法连续入选2018年8月至2018年10月于宝鸡高新人民医院心内科住院拟诊为冠状动脉粥样硬化性心脏病(冠心病)患者153例,常规检测血清LP-PLA2水平,以175 ng/ml为临界值分为对照组(78例)和升高组(75例),行冠脉造影并计算Gensini积分,同时计算冠脉血流校正的TIMI帧数(CTFC),分析LP-PLA2水平与冠脉病变及慢血流发生之间的关系。结果两组在年龄,性别,体质指数(BMI),吸烟,血压,血糖,血脂等冠心病传统危险因素差异无统计学意义(P>0.05),LP-PLA2升高组冠脉病变Gensini积分及冠脉血流CTFC均高于对照组(P<0.01),校正冠心病传统危险因素后,LPPLA2仍是冠脉Gensini积分及冠脉血流CTFC独立的危险因素。结论LP-PLA2可用于冠脉病变严重程度的预测,且在冠脉慢血流发生中具有一定的预测价值。 Objective To investigate the relationship among level of serum lipoprotein-associated phospholipase A2(LP-PLA2),severity of coronary artery lesions and incidence of coronary slow flow(CSF).Methods The patients(n=153)with possible coronary heart disease(CHD)were continuously chosen from Department of Cardiology of Baoji Hi-Teh People’s Hospital from Aug.2018 to Oct.2018.The level of serum LP-PLA2 was detected in all patients,and the patients were divided,taking 175 ng/mL of LP-PLA2 level as critical value,into control group(n=78)and abnormal group(n=75).The patients were received the examination of coronary angiography(CAG),and Gensini integrals and corrected TIMI frame count(CTFC)were calculated.The relationship among LP-PLA2 level,coronary artery lesions and CSF incidence was analyzed.Results The difference in age,sex,BMI,smoking,blood pressure,blood sugar and blood fat(traditional CHD risk factors)had no statistical significance(P>0.05).Gensini integrals for coronary artery lesions and CTFC for coronary flow were all higher in abnormal group than those in control group(P<0.01).After corrected traditional CHD risk factors,LPPLA2 was still the independent risk factor of Gensini integrals and CTFC.Conclusion LP-PLA2 can be applied for predicting the severity of coronary artery lesions,and has some predictive value to incidence of CSF.
