经尿道膀胱肿瘤电切术治疗浅表性膀胱癌的效果

Effects of transurethral resection of bladder tumor in treatment of superficial bladdercancer

目的:探讨经尿道膀胱肿瘤电切术(TURBT)治疗浅表性膀胱癌的效果。方法:选取80例浅表性膀胱癌患者作为研究对象,按随机数字表法将其分为对照组和研究组各40例。对照组采用开腹膀胱肿瘤切除术治疗,研究组采用TURBT治疗,比较两组手术相关指标水平、治疗前后癌症患者生命质量测定量表(EORTC QLQ-C30)评分、并发症发生率、复发率及无进展生存时间。结果:研究组手术时间、导尿管留置时间、术后膀胱冲洗时间及住院时间明显短于对照组,术中出血量明显少于对照组,差异均有统计学意义(P<0.05);研究组EORTC QLQ-C30评分高于对照组,差异有统计学意义(P<0.05);研究组并发症发生率为5.00%,低于对照组的20.00%,差异有统计学意义(P<0.05);两组复发率、无进展生存时间比较,差异无统计学意义(P>0.05)。结论:TURBT治疗浅表性膀胱癌效果优于开腹膀胱肿瘤切除术,能够减轻创伤,降低并发症发生率,促进患者术后恢复,提高其生命质量。

Objective:To explore effects of transurethral resection of bladder tumor(TURBT)in treatment of superficial bladder cancer.Methods:80 patients with superficial bladder cancer were selected as the research objects,and were divided into control group and study group according to the random number table method,40 cases in each.The control group was treated with open resection of bladder tumor,while the study group was treated with TURBT.The surgery-related index levels,quality of life scale of European organization for research and treatment of cancer(EORTC QLQ-C30)score,complication rate,recurrence rate and progression-free survival time before and after the treatment were compared between the two groups.Results:The operation time,catheter indwelling time,postoperative bladder irrigation time and hospital stay in the study group were significantly shorter than those in the control group;the intraoperative blood loss was significantly less than the control group;and the differences were statistically significant(P<0.05).The EORTC QLQ-C30 score of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the study group was 5.00%,which was lower than 20.00%in the control group,and the difference was statistically significant(P<0.05).However,there were no significant differences in the recurrence rate and the progression-free survival time between the two groups(P>0.05).Conclusions:TURBT is more effective in the treatment of superficial bladder cancer than open resection of bladder tumor.It can reduce the trauma,reduce the incidence of surgical complications,promote the postoperative recovery of these patients,and improve their quality of life.
