带线锚钉修复急性跟腱近止点断裂临床效果及并发症分析

Analysis of clinical effect and complication of repairing acute proximal insertion rupture of Achilles tendon with suture anchors

目的分析带线锚钉修复跟腱近止点断裂临床效果及并发症。方法选取2015年5月—2019年5月内蒙古医科大学第二附属医院诊治的30例跟腱近止点断裂患者的临床资料,行回顾性分析,按随机数字表法分为两组,对照组15例行肌腱缝合线缝合术治疗,研究组15例行带线锚钉修复治疗,观察两组疗效、手术时间、并发症发生情况及跟腱功能恢复情况。结果研究组手术时间(35.62±3.83)min,对照组手术时间(68.47±5.92)min,两组比较差异有统计学意义(P<0.05);研究组并发症发生率6.67%,远低于对照组33.33%(P<0.05);术后3个月随访,X线片检查显示锚钉组患者未见锚钉移位和异位骨化等现象。跟腱功能评定研究组患者优良率为93.33%,优于对照组的73.33%(P<0.05)。结论带线锚钉修复急性跟腱近止点断裂的临床效果良好,且能缩短手术时间,术后并发症发生率低。

Objective To analyze the clinical effects and complications of suture anchors in repairing proximal insertion rupture of Achilles tendon.Methods The clinical data of 30 patients with proximal Achilles tendon rupture treated in the Second Affiliated Hospital of Inner Mongolia Medical University from May 2015 to May 2019 were selected for a retrospective case-control study.15 patients in the control group were treated with tendon suture.15 patients in the study group were treated with thread anchor repair.The curative effect,operation time,complications and functional recovery of Achilles tendon were observed in the two groups.Results The operation time of the study group(35.62±3.83)min,control group(68.47±5.92)min,was statistically significant.The incidence of complications in the study group was 6.67%,which was much lower than that in the control group(33.33%).Follow-up 3 months after operation,X-ray examination showed that there was no anchor displacement and heterotopic ossification in the anchor group.According to the functional evaluation of Achilles tendon,the excellent and good rate of patients in the anchor nail group(study group)was 93.33%,which was better than 73.33%in the suture group(control group)(P<0.05).Conclusion The clinical effect of repairing acute rupture of Achilles tendon with wire anchor is good,and the operation time can be shortened,and the incidence of postoperative complications is low.
