整体护理干预对行造口术膀胱肿瘤患者生活质量的影响

Effect of holistic nursing intervention on quality of life of patients undergoing cystostomy

目的对整体护理干预在膀胱肿瘤手术造口患者中的临床意义及生活质量的影响进行调研和分析。方法选择2017年3月—2019年3月某医院肿瘤科接受膀胱肿瘤手术同期行泌尿造口患者93例作为研究对象,以随机数字表法分为对照组(46例)和观察组(47例)。对照组患者接受基础护理干预,观察组患者在对照组基础上采用整体护理干预。观察并对比两组患者护理前后心理情绪、治疗依从性、生活质量以及护理满意度。结果观察组患者的SAS评分为(40.12±3.15)、SDS评分为(32.17±3.26),均低于对照组的(51.29±4.13)、(38.47±3.79),(t值分别为14.685,8.600,P值均<0.05);观察组患者临床治疗的依从率为93.62%,高于对照组的71.74%,(χ^(2)=16.715,P<0.05);观察组患者的认知功能、社会功能、身体状态、家庭作用评分分别为(74.58±9.65)、(78.56±7.65)、(81.27±9.86)、(84.79±9.64),高于对照组的(62.58±7.32)、(64.37±5.21)、(68.54±6.34)、(70.12±7.58),(t值分别为6.746、10.433、7.388、8.146,P值均<0.05);观察组患者的护理满意度为97.87%,高于对照组的84.78%,(χ^(2)=10.814,P=0.001)。结论在膀胱肿瘤患者接受造口手术治疗的过程中,采用整体护理干预的临床效果较好,值得在临床治疗中推广。

Objective To investigate the clinical significance and effect of holistic nursing intervention on quality of life of patients undergoing cystostomy.Methods A total of 93 cases of urologic stoma patients who underwent bladder tumor surgery in the oncology department of a hospital were selected as the research objects,from March 2017 to March2019.They were randomly divided into a control group(46 cases) and a trial group(47 cases).The patients in the control group received the basic nursing intervention,and the patients in the trial group received the holistic nursing intervention on the basis of the control group.The psychological emotions,treatment compliance,quality of life,and nursing satisfaction were observed and compared between two groups before and after the nursing.Results In the trial group,the average score of selfrating anxiety scale(SAS) was(40.12±3.15) points and the average score of self-rating depression scale(SDS) was(32.17±3.26) points,which were significantly lower than those in the control group [(51.29±4.13) points and(38.47±3.79) points respectively,t=14.685,8.600,both P<0.05].In the trial group,the compliance rate of clinical treatmentwas 93.62%,which was significantly higher than that in the control group(71.74%,Xz=16.715,P<0.05).The scores of cognitive function,social function,physical state,and family function in the trial group were(74.58±9.65) points,(78.56±7.65) points,(81.27±9.86)points,and(84.79±9.64) points respectively,which were significantly higher than those in the control group [(62.58±7.32)points,(64.37±5.21) points,(68.54±6.34) points,and(70.12±7.58) points respectively,t=6.746,10.433,7.388,8.146,all P<0.05].The nursing satisfaction in the trial group was 97.87%,which was higher than that in the control group(84.78%),and the difference was significant(P<0.05).Conclusion In the process of bladder cancer patients undergoing stoma surgery,the clinical effect of holistic nursing intervention is better,and it is worth promoting in clinical treatment.
