摘要
目的观察甲状腺癌组织中泛素羧基端水解酶5(UCHL5)的表达水平变化,探讨其与患者临床病理特征及预后的关系。方法92例甲状腺癌患者,取手术切除的甲状腺癌组织为甲状腺癌组,同一患者的癌旁组织(距癌灶边缘>5 cm)为癌旁组。采用实时荧光定量PCR(RT-qPCR)检测两组组织中UCHL5 mRNA表达,分析其与患者临床病理特征的关系,采用Log-rank法比较UCHL5低表达及高表达患者生存率。结果甲状腺癌组织中UCHL5 mRNA相对表达量高于癌旁组织[0.137±0.052比0.086±0.045,t=3.926,P<0.05]。肿瘤直径>2 cm、AJCC分期Ⅲ-Ⅳ期、有淋巴结转移的甲状腺癌患者病灶组织中UCHL5表达水平高于肿瘤直径≤2 cm、Ⅰ-Ⅱ期、无淋巴结转移的甲状腺癌患者(P均<0.05)。UCHL5高表达患者2年、3年甲状腺癌复发率高于UCHL5低表达患者(P均<0.05),UCHL5高表达和低表达患者1年复发率比较,P>0.05。UCHL5高表达患者无瘤中位生存时间少于UCHL5低表达患者(29.6个月比32.8个月,P均<0.05)。结论甲状腺癌组织中UCHL5表达上调,与患者肿瘤直径、AJCC分期、是否淋巴结转移及预后密切相关。
Objective To observe the expression of ubiquitin carboxylterminal hydrolase L5(UCHL5)in thyroid carcinoma tissues and to investigate the relationships with clinicopathological characteristics and prognosis.Methods Ninety-two thyroid cancer patients were selected as the research objects.The thyroid cancer tissues were taken as the thy⁃roid cancer group,and the adjacent tissues(more than 5 cm away from the edge of the tumor)of the same patient were tak⁃en as the paracancerous group.The expression level of UCHL5 mRNA in specimens was detected by real-time fluorescent quantitative PCR(RT-qPCR),and the relationship between the UCHL5 mRNA expression and clinicopathological charac⁃teristics was analyzed.The survival rates of patients with low and high expression of UCHL5 were compared by Log-rank method.Results The relative expression of UCHL5 mRNA in the thyroid carcinoma tissues was higher than that in the adjacent tissues[(0.137±0.052)vs.(0.086±0.045)],with statistically significant difference(t=3.926,P<0.05).The expression levels of UCHL5 in thyroid carcinoma patients with tumor diameter>2 cm,AJCC stageⅢ-Ⅳand lymph node metastasis were higher than those in patients with tumor diameter≤2cm,AJCC stageⅠ-Ⅱand without lymph node me⁃tastasis(all P<0.05).The 2-year and 3-year recurrence rates of patients with high expression of UCHL5 were higher than those of patients with low expression of UCHL5(both P<0.05).There was no significant difference in the 1-year recur⁃rence rate between patients with high CUHL5 expression and patients with low expression of CUHL5(P>0.05).The medi⁃an free survival time of patients with higher expression of UCHL5 was shorter than that of patients with low expression of UCHL5(29.6 month vs.32.8 month),with statistically significant difference(P<0.05).Conclusion The expression of UCHL5 is up-regulated in the thyroid carcinoma tissues,which is closely related to tumor diameter,AJCC stage,and lymph node metastasis and prognosis.
