摘要
目的:探讨应用钛网联合自体髂骨松质骨移植在牙槽嵴裂修复中的临床效果。方法:选取牙槽嵴裂患者13例,联合使用钛网、自体髂骨松质骨修复牙槽嵴裂。术后6个月复诊,观察术区愈合情况并拍摄CBCT观察植骨区的成骨情况。术后6个月通过最低边缘骨水平(MBL)、最小垂直骨高度(SLB)、最小垂直骨宽度(SBW)、植骨存活率及成功率对成骨效果进行评价。结果:术后12例患者植骨区初期愈合良好,植骨区未出现感染、植骨外露等并发症,1例患者因术后2 d伤口裂开感染,导致植骨失败。术后6个月MBL(2.61±0.34)mm,SLB(8.50±0.52)mm,SWB(3.67±0.46)mm,植骨存活率为92.31%,植骨成功率为84.61%。结论:联合应用钛网与自体髂骨松质骨移植修复牙槽嵴裂成功率高,形态功能恢复良好,是修复牙槽嵴裂的可靠方法。
Objective:To investigate the clinical effect of titanium mesh combined with autologous iliac cancel⁃lous bone graft on the alveolar cleft repair.Methods:This study included 13 patients with alveolar cleft,who were treated with titanium mesh,autologous iliac cancellous bone.At 6 months after operation,the healing of the opera⁃tive area was assessed,and the osteogenic effect was observed by cone-beam computed tomography;the osteogenic effect was evaluated by the lowest marginal bone level(MBL),the shortest vertical length of bone bridge(SLB),the shortest anteroposterior bone width(SBW),graft survival rate,and the success rate of grafting.Results:After opera⁃tion,12 cases had good primary healing without infections,graft exposure,and other complications in the operative area;1 case failed due to wound dehiscence and infection at 2 days postoperatively.At 6 months after operation,MBL was(2.61±0.34)mm,SLB was(8.50±0.52)mm,SBW was(3.67±0.46)mm,the graft survival rate was 92.31%,and the success rate of grafting was 84.61%.Conclusion:Titanium mesh combined with autologous iliac cancellous bone graft is a reliable method for alveolar cleft repair due to high success rate and good morphological and functional recovery.
