老年下肢骨折患者行腰硬联合麻醉的安全性分析

Safety analysis of combined lumbar and epidural anesthesia in elderly patients with lower limb fractures

目的:探讨老年下肢骨折患者行腰硬联合麻醉的安全性。方法:2018年5月-2020年5月收治下肢骨折老年患者60例,均接受手术治疗;按照不同麻醉方式分为两组,各30例。对照组实施全身麻醉,观察组实施腰硬联合麻醉,观察两组患者麻醉效果、疼痛程度及不良反应等情况。结果:观察组麻醉药物平均用量、麻醉平均起效时间、术后平均清醒时间均明显低于对照组,差异有统计学意义(P<0.05);观察组术后第3天、第7天疼痛评分均明显低于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:对老年下肢骨折患者行腰硬联合麻醉的效果显著,能减少麻醉药物的用量,减轻患者疼痛程度,降低不良反应发生率。

Objective:To investigate the safety of combined lumbar and epidural anesthesia in elderly patients with lower limb fractures.Methods:From May 2018 to May 2020,60 elderly patients with lower limb fracture were selected with surgical treatment.According to different anesthesia methods,the patients were divided into two groups,with 30 cases in each group.The control group was given general anesthesia.The observation group was given combined lumbar and epidural anesthesia.Anesthesia effect,pain degree and adverse reactions were observed in the two groups.Results:The average dosage of anesthetic drugs,the average onset time of anesthesia and the average postoperative wakefulness time in the observation group were significantly lower than the control group,with the differences were statistically significant(P<0.05).The pain scores of the observation group on the 3rd and 7th postoperative days were significantly lower than the control group,with the differences were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was lower than the control group,with the difference was statistically significant(P<0.05).Conclusion:Combined lumbar and epidural anesthesia is effective in the elderly patients with lower limb fracture,it can reduce the dosage of anesthetic drugs,reduce the pain degree of patients,reduce the incidence of adverse reactions.
