经阴道实时三维子宫输卵管超声造影检查时疼痛影响因素分析

Analysis of factors affecting the pain during transvaginal real-time three-dimensional hysterosalpingocontrast sonography

目的探讨经阴道实时三维子宫输卵管超声造影检查疼痛程度,并分析影响因素。方法选取2018年9月—2020年3月在医院进行3D HyCoSy检查的316例患者,根据患者检查期间是否出现疼痛,将患者分为疼痛组及无痛组,比较两组患者基础情况及检查情况,采用单因素和多因素logistic回归分析导致经阴道实时三维子宫输卵管超声造影检查患者疼痛的因素。结果3D HyCoSy患者,VAS评分为5.19±2.07分,且以中度疼痛患者所占比例最高,患者输卵管通畅程度、造影剂用量较多、球囊较大、伴有紧张情绪及合并妇科炎症均为3D HyCoSy患者出现疼痛感的影响因素。结论3D HyCoSy患者出现疼痛感的危险因素主要包括输卵管通畅程度、造影剂用量较多、球囊较大、伴有紧张情绪及合并妇科炎症等情况,应通过有效评估患者宫颈口径,匀速注入造影剂,调整检查方法,于检查前及检查过程中对患者进行健康指导及心理干预,平复患者情绪及积极治疗患者炎症等方式,降低患者检查期间疼痛程度,保证检查的顺利进行,并提高检查准确度。

Objective To explore the pain degree patients undergoing transvaginal real-time three-dimensional hysterosalpingo-contrast sonography(3D-HyCoSy),and analyze the influencing factors were set as the study subjects.Methods A total of 316 patients who underwent 3D HyCoSy examination in hospital from September 2018 to March 2020 were selected,and according to whether the patients experienced pain during the examination,the patients were divided into a pain group and a painless group,and the basic conditions and examinations of the two groups were compared.According to the situation,single factor and multivariate logistic regression were used to analyze the factors leading to pain in patients undergoing transvaginal real-time threedimensional hysterosalpingography.Results In 3D HyCoSy patients,the VAS score was 5.19±2.07,and the proportion of patients with moderate pain was the highest.The patients'fallopian tube patency,the amount of contrast medium,the larger balloon,the nervousness and the complicated gynecological infl ammation were all the influencing factors for pain of 3D HyCoSy patients.Conclusion The risk factors for pain in patients with 3D HyCoSy examination mainly included the degree of patency of the fallopian tube,the amount of contrast medium,the larger the balloon,the nervousness and the complicated gynecological infl ammation,etc.The patient’s cervical caliber should be effectively assessed,and the patient’s cervical caliber should be effectively evaluated and injected at a constant rate to adjust the inspection method,provide health guidance and psychological intervention to the patient before and during the inspection,calm the mood of the patient and actively treat the infl ammation of the patient,reduce the pain during the inspection,ensure the smooth progress of the inspection,and improve the accuracy of the inspection degree.
