布地奈德联合阿奇霉素治疗小儿支原体感染性肺炎的临床分析

Clinical analysis of budesonide combined with azithromycin in the treatment of children with mycoplasma pneumonia

目的探讨采用布地奈德联合阿奇霉素治疗小儿支原体感染性肺炎的效果。方法选取2016年11月至2017年11月海丰县彭湃纪念医院收治的小儿支原体感染性肺炎患儿94例作为研究对象,将患儿随机归入观察组和对照组,每组各47例,所有患儿均以阿奇霉素进行治疗,观察组加用布地奈德,记录两组退热、喘息消失、咳嗽消失、肺啰音消失、住院时间等临床指标情况;观察两组不良反应情况。结果观察组退热、喘息消失、咳嗽消失、肺啰音消失、住院时间均短于对照组(P<0.05);观察组不良反应发生率较对照组更低(4.26%<17.02%),差异有统计学意义(P<0.05);治疗前,两组患者的肿瘤坏死因子-α(TNF-α)、白介素(IL-6)及超敏C反应蛋白(hs-CRP)水平比较,差异无统计学意义(P>0.05);治疗后,观察组患者TNF-α、IL-6及hs-CRP水平显著低于治疗前,且低于对照组治疗后的水平,差异有统计学意义(P<0.05)。结论布地奈德、阿奇霉素作为治疗小儿支原体感染性肺炎的方案进行治疗,患儿肺部炎症情况显著改善,各项临床症状更快消退,住院时间短,且药物安全性较好,值得推荐。

Objective To investigate the efficacy of budesonide combined with azithromycin in the treatment of children with mycoplasma pneumonia.Methods A total of 94 children with mycoplasma pneumonia admitted to Haifeng Pengpai Memorial Hospital from November 2016 to November 2017 were selected for research.The children were randomly divided into the observation group(n=47)and the control group(n=47).All the children were treated with azithromycin,while the observation group was treated with budesonide.The clinical indexes such as the resolution time of fever,wheezing,cough and pulmonary rales and hospitalization time were recorded,and the adverse reactions(ADRs)of the two groups were observed.Results The resolution time of fever,wheezing,cough and pulmonary rales and hospitalization time in the observation group were shorter than those in the control group(P<0.05).The incidence of ADRs in the observation group was 4.26%,which was lower than 17.02%in the control group,and the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in TNF-α,IL-6 and hs-CRP levels between the two groups of research objects,and the differences were not statistically significant(P>0.05).After treatment,the levels of TNF-α,IL-6 and hs-CRP in patients of the observation group were significantly lower than those before treatment,and they were lower than the levels in the control group after treatment.Meanwhile,the differences were statistically significant(P<0.05).Conclusion Treating children with mycoplasma pneumonia with budesonide and azithromycin,the pulmonary inflammation of children can be improved significantly,the clinical symptoms disappear faster,the hospitalization time is short,and the drug safety is relatively good,which is worth of recommending.
