直接前入路全髋关节置换术对老年股骨颈骨折患者术后康复的效果观察

Observation on rehabilitation efficacy of total hip arthroplasty via direct anterior approach in elderly patients with femoral neck fracture

目的探讨直接前入路全髋关节置换术对老年股骨颈骨折患者术后康复的效果。方法选择2018年3月至2019年10月在黑龙江省牡丹江林业中心医院骨科治疗的84例患者作为研究对象,采取随机数字表法将患者分为对照组和观察组,每组各42例。对照组采取后外侧入路的方式,观察组采取直接前入路方式。治疗后,比较两组患者的手术时间、术中出血量、术后引流液量、患者下地活动时间、Harris髋关节评分和并发症发生率。结果两组患者并发症总发生率比较,差异无统计学意义(P>0.05)。观察组患者手术时间、术中出血量、术后引流液量和患者下地活动时间均少于对照组,术后3个月Harris髋关节评分高于对照组,差异有统计学意义(P<0.05)。结论采取直接前入路手术方式行全髋关节置换术治疗,短期效果明显。

Objective To investigate the rehabilitation efficacy of total hip arthroplasty via direct anterior approach in elderly patients with femoral neck fracture.Methods A total of 84 patients treated in the orthopedics department of Mudanjiang Forest Center Hospital of Heilongjiang Province from March 2018 to October 2019 were selected as the research objects and were divided into the control group(n=42)and the observation group(n=42)according to the random number table method.The control group was treated with posterolateral approach method,while the observation group was treated with direct anterior approach method.After treatment,the operation time,intraoperative hemorrhage,postoperative drainage volume,patients'ambulation time,Harris hip score and complication incidence rate were compared between the two groups.Results There was no statistically significant difference in the total incidence rate of complications between the two groups(P>0.05).The operation time,intraoperative hemorrhage,postoperative drainage volume and patients'ambulation time in the observation group were less than those in the control group,and Harris hip score 3 months after operation was higher than that in the control group,with statistically significant differences(P<0.05).Conclusion Total hip arthroplasty by direct anterior approach has obvious short-term efficacy.
